POSITION SUMMARY: The Discharge Planner RN (Registered Nurse) or LPN (Licensed Practical Nurse) will work with attending physicians, specialists and treatment team to coordinate the patient's transition and discharge following in patient care. ESSENTIAL FUNCTIONS : To perform this job, an individual must perform each essential function satisfactorily with or without a reasonable accommodation; including, but not limited to: Identifies the strengths, needs, abilities, and preferences of each person served Assists in implementing the individual plan of each person served Secures services that are consistent with the needs of each person served through direct interaction with that person and/or with individuals identified by that person Makes initial contact with referral sources and potential patients Assist with coordination of the referral and intake process Determines appropriate level of care, as well as appropriateness of available services Responsible for assisting with maintaining admission reports or screening logs; to include a review of all available information, as well as discussions with referral sources Assist with maintaining a waiting list with continual review and updating of the list, to include how admissions will be prioritized Assist with completing admission paperwork Initiates patient satisfaction surveys Complete discharge packets to ensure patient has pertinent plan of care information Engages patients and responsible parties in the discharge process Documents the condition of the patient at the time of discharge Completes psychosocial assessments, developments treatment planning, and modalities, and ensure those correlates with discharge planning goals as deemed appropriate Assist with post-discharge follow-up to reduce readmissions Reviews community education follow-up for assessing continued service needs Supervises the Admission Coordinators duties when related to discharge planning Offer discharge planning services to the patient in the way of supportive, individual, family, or group therapy focused on discharge goals of the patient. Carry out a liaison role with community resource providers Required Work Experience: Two (2) Years of clinical experience in a psychiatric setting Medical terminology is essential to this position Licensing/Certification/Education Requirements: RN/LPN/MSW with psychiatric nursing experience preferred: or a degree in health-related field from an accredited school. BSW accepted with experience and training Current State of Alabama License RN, LPN, or MSW preferred. POSITION SUMMARY: Wound Care NOW OFFERING DAILY PAY The primary purpose of the Wound Care Nurse us to provide primary skin care to residents under the medical direction and supervision of the residents attending physicians, the Nursing Supervisor, Director of Nursing or the Medical Director of the facility, with an emphasis on treatment and therapy of skin disorders.
