Follow and carry out individualized Plans of Care established by the supervising RN. Perform skilled nursing visits and complete accurate, timely clinical documentation. Monitor patient status and promptly communicate changes to the supervising RN and healthcare team. Educate patients and caregivers on medications, treatments, disease management, and self-care techniques. Reinforce the Home Health Aide plan of care and collaborate with the interdisciplinary team to ensure quality patient outcomes. Participate in case conferences, staff meetings, and quality improvement activities. Promote safe, compassionate, high-quality patient care while helping patients remain safely at home. Qualifications Graduate of an accredited Licensed Practical Nursing (LPN) program. Current, unrestricted Ohio Licensed Practical Nurse license. Current CPR certification. You will evaluate donor reaction(s), which occurs at the facility as outlined in the SOPs. Follow applicable SOPs for medical emergencies including the development of Center Physician's standing orders and donor transport to emergency care facilities. You will refer to the Center Physician or Medical Affairs (as applicable) when in need of providing unacceptable findings to donors, or guidance concerning medical or technical issues, including donor safety and eligibility. You will support the Hepatitis B and Seasonal Flu vaccination programs for employees as applicable. You will manage employee incidents and determine whether further evaluation is required by occupational health/ER. Refer to EHS guidance regarding employee incidents.
