Follow and carry out individualized Plans of Care established by the supervising RN. Perform skilled nursing visits and complete accurate, timely clinical documentation. Monitor patient status and promptly communicate changes to the supervising RN and healthcare team. Educate patients and caregivers on medications, treatments, disease management, and self-care techniques. Reinforce the Home Health Aide plan of care and collaborate with the interdisciplinary team to ensure quality patient outcomes. Qualifications Graduate of an accredited Licensed Practical Nursing (LPN) program. Current, unrestricted Ohio Licensed Practical Nurse license. Current CPR certification.
