Essential Functions Patient Care Provide direct patient care in accordance with the state Nurse Practice Act, including specialized duties related to end-of-life care. Implement current nursing practice following a comprehensive assessment and the established plan of care. Assess and educate patients and their families/caregivers on diagnosis, care needs, and symptom management. Use and prepare equipment and supplies safely and effectively, consistently adhering to aseptic technique. Deliver appropriate pain and symptom management; monitor patient response to treatments and medications. Communication & Coordination Document all care accurately and timely in alignment with the plan of care. Participate in care coordination and promptly report needs requiring involvement from other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the RN and/or Clinical Care Manager. Additional Responsibilities Participate in ongoing personal and professional development. Contribute to quality assessment and performance improvement (QAPI) activities. Perform other duties as assigned by the Registered Nurse. Qualifications Graduate of an accredited practical nursing or vocational nursing program. Minimum of three years of nursing experience preferred; home health, community-based, or medical-surgical experience ideal. Current state LPN/LVN licensure. Current CPR certification. Graduation from an accredited School of Practical Nursing. 2. Must hold an active license in the state as a Licensed Practical Nurse 3. One, or more, years of experience, in community/home health agency setting, is preferred. 4. Must have a criminal background check. 5. Must have current CPR certification. Online certification is not accepted 6. The ability to communicate well, both, verbally, and in writing, in English, is required. 7.Bilingual (English/Spanish) strongly preferred Environment: Contact with clients under a wide variety of circumstances. Care is provided in client’s living environment with varying situations, including private homes, independent or assisted living or in an institution such as a hospital or long-term care facility. May be required to respond to emergency situations. Travel required. Position includes tasks that have the potential for exposure to blood/body fluids/tissues and other potentially infectious material such as body fluids/tissues. Physical Requirements: Must be able to stand, walk, squat, bend, kneel, reach, twist, push and pull. Must be able to lift up to 50 lbs. Corrected visual and hearing acuity required.
