Provide explanation of nursing procedures and assistance to staff to assure ongoing understanding and training. Communicate on a regular basis with the Nurse Manager or Director of Nursing regarding observations, and suggestions of problem areas, resident and employee activities, concerns, needs, conditions, etc. Observe and evaluate residents' health, mental and emotional conditions and communicate significant changes to appropriate staff. When indicated is responsible for notifying the physician and family members according to prescribed procedures. Provide direct patient care as appropriate including, but not limited to, procedures such as catheter placement and care, colostomy care, suction, dressings, naso-gastric tubes and other treatments. Participate in report at the beginning and end of shifts to assure proper information about residents is communicated. Assist in the admission, orientation, and observation of new residents, assure that physician orders for diets, therapy, medication, etc. are transcribed and communicated according to procedure. The ideal candidate will have: Graduate of an approved school of licensed practical/vocational nursing. Current license, in good standing, as an LPN/LVN in the State. One year nursing experience. Complete required assessments upon the customer’s admission, transfer, and/or discharge. Receive telephone orders from physicians and record on the Physicians’ Order Form and P.O.S. Record new/changed diet orders. Forward information to the Dietary Department. Perform routine charting duties as required and in accordance with our established Charting and Documentation Policies and Procedures. Responsible for completion of admission, discharge, and transfer processes. Prepare and administer medications and treatments if appropriate as ordered by the physician. Review medication record for completeness of information, accuracy in the transcription of the physician’s order, and adherence to stop order policies. Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, care of the dead/dying, etc., as required. Participate in the orientation of new customers/family members to the facility. Arrange for diagnostic and therapeutic services, as ordered by the physician. Notify the customer’s attending physician and family when there is a change in the customer’s conditions. Obtain sputum, urine and other lab tests as ordered. Take and record TPR’s, blood pressures, accuchecks, etc., as necessary. Contact the customer’s physician for: Nursing assessment of change of condition. Medication orders or charges. To inform of incidents. To report abnormal results of Lab and X-ray. Anything else pertinent to the customers’ care and well-being. Ensure the nurses’ notes and monthly summaries reflect the care plan and is being followed. Attend staff meetings, etc., as directed. Perform other related duties as assigned. Coordinates interdisciplinary participation in completing the MDS for each resident according to regulatory time frames. Ensures completeness and thoroughness of documentation as mandated by federal and state standards. Schedules and conducts resident care conferences in compliance with state and federal regulations and ensures completion of all MDS reviews prior to resident care conference. Assists disciplines in formulating and revising care plans. Ensures that resident’s present/potential problems are identified and prioritized; realistic goals are established and nursing intervention is appropriate. Evaluates resident care plans for comprehensiveness and individuality. Assesses the achievement or lack of achievement of desired outcomes. Ensures that resident’s care plan is reassessed and revised appropriately. Responsible for all level of care changes within the facility. Notifies all departments when a level of care change has been made. Generates appropriate forms to complete level of acuity and changes. Transmits forms to the appropriate agency for processing as required by state law. Other duties as assigned.
