Conducting Physical Assessments: -Assist all two center base sites with health related information for each child and review all medical information and charts. -Document visits at all sites for data collection in accordance with all guidelines for health- related child outcomes. -Review child physicals upon enrollment and on a regular basis thereafter to ensure appropriate updates and sustaining of ongoing physical updates. -Ensure all physicals have been completed within the last year. -Ensure the child is up to date with vaccinations. -Provide guidance for the completion of Child Health Care Plans and monitor every six months for update compliance, renewals, or changes in medication and to update doctor/parent permission to continue administering. -Review of any medications that are related to allergies or specific medical care instructions and note this information in the children's file. Professional Responsibilities: -Complies with medical, nursing, and educational ethics. -Exhibit strong organizational and administrative skills to implement and promote the health program. -Case management, health intervention and interdisciplinary collaborating skills. -Awareness of community resources and referral information. -Advanced skills in interpersonal communication and health counseling. -Implement advanced nursing protocols on pediatrics and adolescent medicine . The LPN supervises the GCT and CNA, who report directly to the LPN. Operations Observes, records, and reports resident data according to policy and procedure. Administers resident care as assigned by the registered nurse within the Nursing Department guidelines . Participates in giving direct resident care. Assumes the responsibilities under the direction of a registered nurse associated with the role of a charge nurse or associated primary nurse, as the neighborhood position requires. Directs and supervises non-professional staff as assigned. Follows all personnel policies and procedures (i.e., attendance, dress code) of the facility . Ensures that appropriate documentation concerning unauthorized discharges is entered in the resident’s medical record in accordance with established procedures. Performs routine charting duties as required and in accordance with our established charting and documentation policies and procedures. Orders prescribed medications, supplies and equipment as necessary, and in accordance with facility-established policies Notifies the nurse manager/charge nurse of all drug and narcotic discrepancies noted. Develops work assignments and/or assists in completing and performing such tasks. Ensures that department personnel, residents, and visitors follow the department’s established policies and procedures at all times. Documents resident care provided following the nursing process and as per the facility and Nursing Department’s documentation policy and procedures. Clinical Participates in the formulation, implementation, and evaluation of resident care plans to meet the particular needs of the geriatric resident under the supervision of the registered nurse. Administers medications and performs nursing procedures and treatments according to policy and procedure. Reviews medication administration records for completeness of information, accuracy in the transcription of the physician’s order and adherence to stop order policies Receives telephone orders from physicians and enters the same on the EMR. Signs off physician’s orders on the EMR and transcribes orders to resident’s care plans as required and as per policy and procedure. Prepares and administers medications as ordered by the physician. Ensures that prescribed medication is administered following the 5 rights in medication administration. Ensures that narcotic records are accurate Reviews medication orders for completion to assure that medications are being administered as ordered and treatments are provided as scheduled Employee Engagement Fosters a positive working environment, promoting teamwork and staff development Addresses employee concerns and ensures that staff adhere to organizational policies.
