Participate in care coordination and promptly report needs requiring involvement from other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the RN and/or Clinical Care Manager. Perform other duties as assigned by the Registered Nurse. Qualifications Graduate of an accredited practical nursing or vocational nursing program.
