Job Description This position leads resources over Diabetes Education, Palliative Care and Primary Stroke initiatives and works to ensure regulatory requirements are met. The Manager has major responsibility for the implementation of the vision, mission, philosophy, core values, evidence-based practice, and standards of the organization. Responsibilities Overseeing the overall Palliative Care, and Primary Stroke programs, management, implementation of the programs and creating desirable outcomes. Implement short term and long-term goals for program development and economic success. Cultivate existing relationships with various interdisciplinary team members through communication, active participation in house wide groups / initiatives in alignment with program goals. Manage the data collection and reporting requirements for the program. Maintain and oversee all the requirements for the Joint Commission’s Disease Specific Care (DSC) Certification for Primary Stroke Care Program, Palliative Care, Diabetes Education Services. Maintain and oversee all the requirements for the American Diabetes Association (ADA), Accreditation for outpatient sites. Administrative Leadership Leads performance improvement initiatives in assigned areas to improve outcomes. Demonstrates knowledge of job standards and key requirements for each staff position in assigned areas of responsibility, holding subordinates accountable for the responsibilities inherent to their positions. Provides feedback/counseling, and discipline to assigned staff as appropriate. Provides assigned staff with performance evaluations as defined by HR. Supports peer review systems and implements as feasible with mechanisms in place for staff input into their own evaluations. Provides structure and opportunity for staff to establish and accomplish their goals. Delegates appropriate responsibility and accountability for designated management activities. Conducts regular leadership and department meetings. Creates an atmosphere that encourages staff participation in nursing and hospital-based activities. Seeking out opportunities for and supports community service activities. Qualifications EDUCATION: Graduate from an accredited School of Nursing. Bachelor’s degree required. MSN Preferred. LICENSES & CERTIFICATIONS : Graduate of an approved School of Nursing and Current RN licensure in the state of employment. The RN holds a current license to practice nursing in her/his state of employment. Membership in professional organizations is desirable. Certification in area of specialty is desirable. BLS certification is required prior to patient contact. YEARS OF EXPERIENCE: Minimum of three (3) years in clinical nursing practice. One (1) year experience in leadership position. REQUIRED SKILLS AND KNOWLEDGE: Demonstrated knowledge of nursing, healthcare, and organizational systems, and strategies. Demonstrated knowledge and experience in performance improvement theory and application. Knowledge of TJC standards and other external organizations related to healthcare evaluation and accreditation. Experience with the methodologies and processes related to organizational financial viability. Well developed communication, facilitation, and interpersonal skills.
