Advanced Practice Provider (APP) - Cardiothoracic Surgery

Job Summary The Cardiothoracic Surgery Advanced Practice Provider (APP) functions primarily in the operating room (OR), consults, clinic, and acute care settings. The APP assists in evaluating and treating patients in step-down, floor, clinic, and consult areas, documenting all care and assessments. Job Description A bit more about this role: This position represents an amazing opportunity for an endocrinology or diabetes specialized nurse practitioner (APRN) with a big heart and entrepreneurial spirit. Your primary focus will be delivering world class diabetes care to our members. In this role you will be working in a next generation virtual diabetes clinic that dramatically expands access to specialty care for America's most vulnerable seniors. You will also utilize and help improve our home-grown technology and electronic health information platform to carry out these visits. On a day-to-day basis you will work closely with our virtual speciality clinic team members at Devoted Medical including Certified Diabetes Care and Education Specialists (CDCES), physicians and other APRNs, medical assistants, pharmacists and social workers. You will be a key member of our interprofessional team. The diabetes clinic is one of several of Devoted's virtual specialty care programs that were designed as “micro centers of excellence” that deliver highly tailored, specialized care to patients with a specific chronic condition - e.g. diabetes, congestive heart failure, hypertension, etc. Responsibilities will include: Conduct focused and thorough assessments of patients with diabetes through virtual consultations including ordering advanced diagnostics, interpreting lab and imaging data, and developing a treatment plan in collaboration with the speciality care clinic team. Formulate accurate diagnoses and develop individualized treatment plans for patients with diabetes, including medication management, lifestyle modifications, and monitoring recommendations. In this role you will be working in a next generation virtual diabetes clinic that dramatically expands access to specialty care for America's most vulnerable seniors. The Psychosocial Oncology Program (POP) is a team of multidisciplinary professionals working with children and families affected by cancer. Commonly known as the POP Team, our mission is to help children, teens, young adults, and their families navigate emotional, social, and cognitive concerns after cancer diagnosis. The UI Stead Family Department of Pediatrics is seeking an APP to join our division of Hematology-Oncology as a member of the POP Team. In this role, the APP works in an integrated care model with the medical oncology team to assess and facilitate management of mental health aspects of pediatric cancer care. POP APPs perform comprehensive biopsychosocial assessments for children following cancer diagnosis and periodically throughout the cancer trajectory. Based on assessment findings, the APP facilitates appropriate referrals and connection to several psychosocial resources. The APP provides therapeutic support with all visits and, when necessary, prescribes and manages psychiatric medications as an intervention.
