Description The APN/NP is a specialty health care provider who provides medical and nursing services to individuals and families. The APN/NP is adept at diagnosing and managing acute and chronic illnesses, and emphasizes health promotion and disease prevention. Develops medical treatment plans based on scientific evidence-based practice and evaluates outcomes. Mentors staff/students in aspects of advanced nursing practice and patient care Conducts comprehensive appraisals of patient health status by taking health histories and performing physical examinations. Initiate and participate in admission histories and physicals, medical consultations in related subspecialty, and discharge planning. Order diagnostic tests and procedures. Establish a diagnosis based on the history, physical examination and diagnostic test results. Formulates differential diagnosis. Evaluate results of diagnostic testing of laboratory tests, EKG’s imaging studies, and medical procedures. Make appropriate consultations and referrals. Develop a treatment plan based on diagnosis and evidence-based practice that is mutually acceptable by the patient and family. Determine the effectiveness of the treatment plan with documentation of patient care outcomes. Reassess and modify the treatment plan to achieve optimal health and medical goals for the patient. Teaches and counsels families in medical conditions, treatment plans, preventive care and anticipated outcomes.
