Description A nurse practitioner is certified under section 6910 of the New York State Education Law and licensed as a registered nurse. A nurse practitioner provides acute/primary health care in accordance with written policies approved by nursing and medical administration. Responsibilities 1. Develops and implements a collaborative practice agreement in the specific clinical area and performs a periodic review of the collaborative agreement. 2. Assesses the health status of patients/families by means of comprehensive health history and physical examination. 3. Initiates selected therapeutic and diagnostic procedures, consultations and laboratory examination, prescribes and alters medication regimens, interprets findings and alters plan of care according to collaborative practice agreement and protocols in designated area. 4. Clinical Program Manager-Geriatric and Palliative Medicine

Description The Clinical Program Manager for ALIGN-CARE Geriatric Surgery Co-Management Program is responsible for overseeing the continuity of patient care. Patients will be 65 years or older, frail, community-based, considering surgery, assessing surgical risks and provide care plans to optimize health prior to surgery. Patients enrolled in the program may come from any part of Mount Sinai Health System; however, hospitals of care receipt will be both Mount Sinai Hospital and Mount Sinai Morningside Hospital. Responsibilities Oversees clinical operations within the program, using data and fact-based problem-solving techniques to improve processes and outcomes. Ensures that all clinical initiatives run according to institutional and departmental policies and in accordance with any government or regulatory requirements as applicable. Plans, develops, and/or maintains departmental standard operating procedures. Regularly reviews policies and procedures and makes changes or recommends changes to superiors as necessary. Description The Clinical Program Manager provides clinical support and administrative direction within the Tisch Cancer Center’s Clinical Trials Office (TCC CCTO). In collaboration with the Associate Director for Research, clinical trials investigators, and TCC CCTO Administrative Directors; this candidate is responsible for the implementation and conduct of clinical trials with investigational anti-cancer agents in the Cancer Institute. The Candidate must have expert knowledge of cancer as a disease process, cancer treatment modalities, and the process of conducting clinical research. The Candidate will follow a caseload of patients in the ambulatory setting and assist in the administration and plan of the research program. The Clinical Program Manager will report into the CCTO’s Administrative Director. Responsibilities Administrative Responsibilities: Supervises Clinical Research Coordinator positions and other CRNs to ensure protocol compliance, accurate data collection and sample acquisition.
