Overview This individual is a clinically competent Cardio-Vascular Advance Practice Clinician (APC) will directly report to the Director of APCs with a matrix reporting structure to the Executive Director of the Cardiac and Vascular Service Line. S/he must be able to effectively deliver care through demonstrated expertise to the Cardio-Vascular patient. The principle duties and responsibilities include but are not limited to: performing pre-procedure assessments, Histories and Physicals, education and counseling of patients on cardiac and vascular diseases and treatments, responsible for tentative diagnosis, order and provide preliminary interpretation of lab studies, post procedure management of patient, discharge patient. Responsibilities Responsibilities Provides timely and thorough pre procedural work up on all Cardio-Vascular patients to include orders, History and Physical, review of consent and lab tests. Orders and collects data using appropriate evidence-based assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated. Assesses for risks associated with the care of the acute and complex chronically ill patient including: medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions and diagnostic procedures. Diagnosis Collaborates with interdisciplinary health care team in making diagnoses of acute and complex chronic conditions. Manages diagnostic tests through ordering, interpretation, performance, and supervision. Formulates differential diagnoses by priority. Diagnoses complications and orders appropriate interventions Formulates Plan of Care Identifies expected outcomes from diagnosis, and formulates and documents a plan of care to address complex acute and chronic health care needs. Overview This individual is a clinically competent, certified advanced practitioner, who serves in the comprehensive care of the medical oncology and hematology patient throughout the continuum of care. Under the scope of an advanced practice clinician and approve privileges within Beebe Healthcare, the individual is expected to perform comprehensive patient assessments, follow-up care, diagnosis, side effect and infusion reaction management, survivorship care services, and patient education/counseling in collaboration with the medical oncology physicians.
