As we continue to enhance our general Cardiology services, this position will help support comprehensive inpatient coverage at Wentworth-Douglass Hospital from 7:00 AM to 5:00 PM daily. The service will include four Advanced Practice Providers (APPs) who rotate between the inpatient service and our outpatient Cardiology practices located on Rollinsford Road in Dover and Terrascape Parkway in Somersworth. This integrated model is designed to promote continuity of care for our Cardiology patients across both the inpatient and outpatient settings. This is a 40-hour-per-week position with scheduling flexibility, including the option to work four 10-hour shifts or five 8-hour shifts. The position also includes a weekend rotation of approximately one weekend in four. We are seeking an experienced provider who values autonomy, flexibility, collaboration, and teamwork and is committed to delivering high-quality, compassionate cardiovascular care. This is an excellent opportunity for a motivated Nurse Practitioner or Physician Assistant to make a meaningful impact while working alongside a supportive and innovative Cardiology team. Summary The Nurse Practitioner (NP) is a licensed provider. The NP is responsible for assessing and managing various patient populations, including those with medical and/or surgical conditions, as well as providing diagnostic and therapeutic interventions, developing appropriate plans of care, and conducting ongoing evaluations. May also perform additional duties, such as precepting a small group of learners. Does this position require Patient Care? The Interventional Radiology service operates Monday through Friday from 8:00 AM to 5:00 PM, with no weekends, holidays, or on-call responsibilities. This hire will work collaboratively with a team of Interventional Radiologists while practicing with a high degree of autonomy in the evaluation and management of patients undergoing image-guided procedures. Responsibilities include performing biopsies, paracenteses, thoracenteses, lumbar punctures, and port insertions, as well as providing peri-procedural care and participating in inpatient rounding. NP ROLE AND RESPONSIBILITIES: Provide primary and preventative care for long term care patients residing in skilled nursing facilities. Manage Residents with acute change in condition. Advanced care planning and chronic care management. Prescribe appropriate medications, including controlled substances in accordance with NH Law. Order, review and/or follow-up with treatments and diagnostic testing. Communicate with the nursing home staff on a regular and as needed to discuss changes in condition. and/or the care plan. Collaborate with primary care physician. NP QUALIFICATIONS AND EDUCATION REQUIREMENTS Current, unrestricted NP license in the state of New Hampshire. Board Certification. DEA licensure. Clinical background in adult, family or geriatric specialties. Experience working in a long-term care setting. Ability to work with an EMR. Ability to manage patients with advanced illness and multiple chronic conditions. Ability to work within an interdisciplinary care team to coordinate patient care. WOUND NP ROLE AND RESPONSIBILITIES: Provide wound care services for long-term care patients residing in skilled nursing facilities. Prescribe appropriate medications, including controlled substances in accordance with NH Law. Order, review, and/or follow-up with treatments and diagnostic testing. Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers. Must be comfortable staging pressure injuries according to the NPIAP Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc. Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices Strong preference to having wound certification or be open to getting certified Experience working in a long-term care setting. Ability to work with an EMR. Ability to work within an interdisciplinary care team to coordinate patient care.
