Location: METROHEALTH MEDICAL CENTER Biweekly Hours: 80.00 Shift: 8:00 AM TO 5:00 PM The Department of Emergency Medicine of the MetroHealth System, a principal teaching hospital of the Case Western Reserve University (CWRU) School of Medicine, is seeking Advance Practice Registered Nurse to join our growing team at our Life Flight team. Delivers direct patient care consistent with the advanced practice registered nurse’s education, experience and certification, in collaboration with a physician, which includes diagnosis, management and/or referral of health care issues. Promotes consumer, family, and community health through preventative health care actions and through patient advocacy. The advanced practice registered nurse may also manage complex illnesses and/or deliver specialty care. Upholds the mission, vision, values, and customer service standards of The MetroHealth System. Metro Life Flight: Responsible for advanced emergency and critical care patient management during air and ground transport of the critically ill or injured patient from the scene or outlying sites to participating secondary and tertiary care institutions. Provides intensive care monitoring, critical care intervention and advanced surgical skill procedures to critically ill and injured patients. This includes the management of high-risk obstetrical, medical, surgical, respiratory and cardiovascular patients. The specialized training of the Flight Nurse Practitioner allows for the provision of care to critically ill and injured patients through the use of ventilatory support, continuous cardiac and hemodynamic monitoring, including intra-aortic balloon pump. Reporting safety concerns, errors and near misses. c. Participating in system improvement initiatives to prevent errors. d. Providing evidence-based care as applicable to the role. Clinical Practice Performs patient assessment, documents and analyzes data relative to specialty based on patient needs, existing signs and symptoms and current patient data. Utilizes critical thinking in formulating differential diagnosis and establishing priorities in care. Institutes measures to promote and maintain health, prevent illness, maintain comfort and meet achievable patient and family care preferences. Develops individual, evidence-based plan of care for patient with ongoing management and evaluation of patient outcomes based upon advanced practitioner’s scope of practice and certification in collaboration with patient, family, and/or other pertinent health care provider(s) including collaborating and specialty physician(s) as needed. Provides care in a manner which enhances patient satisfaction; maintains and promotes good public relations. Selects appropriate communication approach to achieve desired outcomes; identifies barriers to communication; initiates appropriate interaction and evaluates results. Accepts accountability for clinical practice including making decisions proactively, responding to changing needs of patients/families and representing patient needs to others as appropriate. Ensures that compliance is maintained with all applicable regulatory agencies and laws, national standards, The MetroHealth System Code of Ethical Conduct and American Nurses Association (ANA) Code of Ethics. Prescribes drugs and therapeutic devices in accordance with laws and within scope of practice, if holds Certificate to Prescribe. For Clinical Practice of the Metro Life Flight Nurse Practitioner: Understands flight physiology and takes appropriate action regarding patient care. Works collaboratively with the Nurse Manager, Metro Life Flight Professional Practice Committee (Shared Governance) and assistant medical directors in the evaluation and improvement of clinical practice. Assures availability via provided hospital pager for programmatic needs including but not limited to: patient care, specialty transports, and other administrative direction. Performs a prehospital scene survey to ensure safety of patient and crewmembers and coordinates patient triage in the pre-hospital environment as needed. Evaluates the care of patients within the scope of flight nurse practitioner utilizing Metro Life Flight protocols/guidelines and physician collaboration and consultation. Maintains current knowledge of transport medicine standards and responsibilities. Attends a minimum of 60% of scheduled staff meetings and documents meeting minutes have been read if not in attendance. Attends a minimum of 50% case review conferences. Attends minimum of two surgical skills lab a year. Attends Operating Room or clinical experience if privilege requirements have not been met. Perform techniques and medical procedures, via human patient simulators, as required to maintain skill levels. Meets all clinical practice, safety, customer service and flight operations needs set forth by the administration of Metro Life Flight and The MetroHealth System. Coordination of Care a. Consults with other professionals for joint problem solving and setting of mutual goals related to patient care issues when necessary. b. Functions as a resource for other nurses and healthcare providers for clinical practice issues and provides education as needed. c. Plans for continuity of care, identifies community health resources, guides patients and families in their use, and facilitates continuity of care on transfers as needed acting as liaison between patient/family/physician/community resources as needed. Leadership/Organizational Skills a. Participates in achieving division/department goals; communicates division/department-based issues and concerns in an appropriate forum. b. Participates or supports advance practice registered nursing initiatives in The MetroHealth System. c. Participates as assigned in the orientation of new staff and serves as a resource and/or preceptor for students from various disciplines, and new personnel as well as other disciplines as appropriate. d. Serves on committees and/or participates in professional activities within the department, institution and/or community. e. Presents formal educational programs. For Life Flight Nurse Practitioner: required to present or teach a minimum of two (2) medical educational programs per year to a hospital or EMS/Community organization. Professional Development a. Remains current with evidence based practice, adhering to national standards and ANA Code of Ethics. b. Identifies own learning needs. Sets learning goals, and actively pursues required knowledge and skills. c. Evaluates own professional practice utilizing self, collaborative physician, and peer evaluations according to state law. Includes patient and /or family feedback. d. Participates in performance improvement activities focusing on improving patient care outcomes. e. Participates in the identification of research projects; evaluates and analyzes research that is available in the literature for possible integration into daily practice in support of evidence-based practice. Qualifications: Required: MSN or grandfathered under Ohio Revised Code Graduate of an accredited advanced practice registered nurse program with acute care licensure (AGACNP or ACNP). Current registered nurse licensure in the state of Ohio Certificate of Authority as an Advanced Practice Nurse from the Ohio Board of Nursing Certification as Advanced Practice Registered Nurse in specialty. Current Basic Life Support (BLS) from the American Heart Association or obtains within 90 days of hire. Trauma and Emergency General Surgery Advanced Practice Provider (APP) Position Overview: The Trauma and Emergency General Surgery Advanced Practice Provider (APP) delivers direct patient care consistent with the APP’s education, experience, and certification, in collaboration with the attending Trauma and Emergency General Surgery physicians. This includes the diagnosis, management, and referral of health care issues specific to Trauma and Emergency General Surgery (EGS). The APP will complete pre- and post-surgical management and recognize options for non-surgical management of trauma, EGS, and critically ill patients. Key Responsibilities Collaborate with Trauma/EGS attending physicians, general surgery residents, and APPs to complete daily rounds on the trauma and surgical floors and in the intensive care unit (ICU). Respond to Emergency Department (ED) consults for EGS, collecting data and interpreting laboratory and imaging results to develop and document assessments and care plans. Perform bedside procedures for both surgical and non-surgical patients Work with the team to complete timely documentation and carry out care plans effectively. Partner with Trauma/EGS APPs, Trauma/EGS attendings, surgical residents, nursing, physical, occupational and speech therapies, social work and case management to develop disposition plans with a multidisciplinary team approach. Advanced clinical practice in nursing includes the following components: assessment of patient health status, diagnostic testing, diagnoses, development of treatment plans, implementation of treatment plans, follow-up and evaluation of the patient’s response to treatment, accurate documentation of the patient’s status, and patient advocacy. APRNs are clinical experts who design, implement and evaluate patient-specific and population-based programs of care and promote care through a variety of methods. APRNs function at all levels of the healthcare system within three sphere of influence: client, personnel, and organization. APRNs must have a collaborating physician(s) in an associated Standard Care Arrangement. The APRN II must spend 50% of hours worked utilizing prescriptive authority in their advanced clinical practice role. Job Requirements Master's Degree in Nursing Graduate from NP, CNS or midwife program Specialty certification in area of expertise - Psychiatric NP Registered Nurse with APRN Licensure Demonstrated expertise in area of clinical specialization Professional membership in specialty nursing organization required within one year of start date 3-4 years experience Clinical in Nursing Nursing practice in geriatric, occupational health, or other applicable setting. Job Responsibilities Collaborates and consults with the interdisciplinary healthcare team, the patient and family when diagnosing, prioritizing problems, and implementing interventions. Provides consultation and initiates referrals to other healthcare team members and promotes collaboration among other members of the interdisciplinary healthcare team. Assists interdisciplinary team members, patients, and families in selecting therapies that integrate perspectives of cost and quality of benefits. Collaborates with other disciplines in teaching, consultation, management, and research activities to improve outcomes in nursing practice and enhance the healthcare environment. Provides effective leadership. Integrates analysis of cost and benefits in care decisions. Designs/Develops innovative solutions for patient care problems that utilize resource efficiency while maintaining or improving quality and advancing best practices. Provides leadership in the development and application of consensus derived protocols or evidence-based practice (EBP) guidelines in the clinical setting. Participates, as opportunities arise, in healthcare-related decision-making at the system, local, state, and national levels with an emphasis on improving patient outcomes. Serves in key roles by participating in/leading committees, councils, and administrative teams. Utilizes, participates in, and disseminates research, evidence-based practice, or quality improvement projects to enhance clinical practice and patient outcomes. May serve as primary or co-investigator, or member of the investigative team in conducting nursing or collaborative research. Critically evaluates existing practice based on current research findings and integrates changes into practice through an EBP project. Evaluates processes of care by serving a as leader or team member in relevant Quality Improvement projects. Promotes mentoring and education to nursing by serving as a Journal Club mentor and facilitates learning in critically appraising the literature. Provides education to patients, families, and healthcare providers to promote best practice. Serves as a teacher, mentor, role model to nurses in the promotion of nursing research, EBP. Serves as preceptor, role model and mentor to staff nurses and colleagues as well as nursing students. Teaches, coaches, and supports nurses and other healthcare professionals to advance the care of patients. Provides education to the patient and their family for health promotion including secondary prevention strategies. Collaborates in the assessment, development, implementation and evaluation of patient and staff educational needs. Ensures nursing practice is conducted in an ethical and legal manner. Abides by the Ohio Scope of Practice as an APRN. Provides safe, compassionate, competent, and ethical care. Demonstrates the highest level of accountability for professional practice. Preserves the patient’s dignity. Serves as a patient advocate by promoting advanced directives and adherence to the patient's wishes. Contributes and promotes the discussions of end-of-life decisions and palliative care with the patient, family members, physician and the health care team as appropriate. At least 50% of position involves direct management of the patient’s and/or family’s care. Independently performs and documents a complete history and physical examination, including psychosocial assessment. Prescribes drugs and therapeutic devices consistent with the formulary under section 4723.50 of the Ohio Revised Code. Assesses for interactive and synergistic effects of multiple pharmacologic and non-pharmacologic interventions. Orders laboratory and radiologic tests and interprets the results. Collaborates with physician(s) to plan to implement a comprehensive plan of care. Discusses patient’s care with family members. Performs duties per the Standard Care Arrangement.
