This position will provide comprehensive, high-quality primary, episodic and/or recuperative care at our main campus and surrounding shelter-based clinics. The ideal candidate for this role must be a self-starter and have the ability to lead a care team in partnership with other clinical peers. Our practice is collaborative, supportive and innovative. Responsibilities: Provides comprehensive, high quality episodic and/or recuperative care at various clinical sites staffed by Boston Health Care for the Homeless Program. May work at shelter-based clinic, hospital-based clinic, respite facility or on street-based outreach based on program needs. Elicit an appropriate history, perform physical examinations, order diagnostic studies, interpret results and initiate therapy in accordance with the Guidelines for Physician Assistants and Nurse Practitioners of Boston Health Care for the Homeless Program. Refer patients as appropriate to medical specialists, hospital admission, recuperative care, and detox facilities, providing appropriate information for continuity of care. Participate in collaborative relationship with BHCHP physician according to the Regulations of the Commonwealth of Massachusetts. Provide education to patients on health promotion and disease treatment and prevention. Work as part of multi-disciplinary team (medical, nursing, dental, BH, pharmacy, case management) in caring for patients. Administrative Responsibilities: Maintain adequate, up-to-date medical records, completed in a timely manner, according to the guidelines of BHCHP, employing standard forms such as problem lists, medication lists, etc. Maintain active DPH, DEA, and professional license, managed care credentialing and CPR training. Some on-call and holiday coverage required. Qualifications: Licensed by the Commonwealth of Massachusetts. Registered or eligible to be registered with the Massachusetts Department of Public Health and the Federal Drug Enforcement Agency. At least one year of direct NP/PA experience. Life support training certification (e.g. CPR, BLS, ACLS) from a course that includes a hands-on, in-person component. If not certified prior to start date, applicant must submit certification within 90 days of start date. Experience in working with underserved populations, mentally ill, homeless and/or persons with substance use disorders preferred. Bilingual proficiency preferred. Position Summary The Inpatient Gastroenterology Consult Nurse Practitioner (NP) is an integral member of a busy inpatient gastroenterology consult service caring for children with a wide range of gastrointestinal, nutritional, feeding, and enteral access needs. Working closely with GI faculty, fellows, residents, medical students, nurses, dietitians, and other members of the multidisciplinary team, the NP evaluates and manages hospitalized patients across Boston Children’s Hospital. The GI consult service provides exposure to a broad and complex spectrum of pediatric gastroenterology, offering an opportunity to develop clinical expertise alongside experienced colleagues and trainees. Time Type: Full time Department: Family Medicine All Locations: 20 Maverick Square – Cradock Building Position Summary: The Advanced Practice Provider (APP) is a licensed independent practitioner who provides medical services to individuals, families and groups. An APP diagnoses and manages acute episodic and chronic illnesses while emphasizing health promotion and disease prevention. Services may include ordering, conducting, supervising and interpreting diagnostic and laboratory tests, and prescription of pharmacologic agents and non pharmacologic therapies. Essential Duties & Responsibilities: Obtains comprehensive and problem focused health history Performs a comprehensive and problem focused physical exam Orders and interprets screening and diagnostic tests Demonstrates critical thinking and diagnostic reasoning skills in clinical decision making.
