As a Care Coordinator, you will work collaboratively with multidisciplinary caregivers across the continuum of care to provide coordination of care and disease management longitudinally to patients with chronic condition(s). This is a regular PRN position, with caregivers working days between 8:00 a.m. – 5:00 p.m. with on-call, weekend and holiday requirements. A caregiver who excels in this role will: Work collaboratively with a multidisciplinary care team across the continuum of care for high-risk patients to develop goals, plan interventions and maximize patient outcomes. Provide care and disease management coordination. Identify patients in the specialty care practice that have ongoing coordination needs and conduct targeted outreach. Outline the nature and duration of involvement needed by the specialty care team and specialty care coordinator and identify the primary care team involved. Utilize assessment skills and risk assessment tools to identify patients with actual or potential care needs that would require care coordination. Utilize technological tools (registries, patient lists, care team tab, etc.) to manage populations. Conduct comprehensive clinical assessments that include disease-specific, age-specific, medical, behavioral, pharmacy, social and end of life needs of each patient. Inform and work with patients and their families regarding coordination of their care, provide education and coaching, monitor patient compliance with their care plan, perform reassessments regarding patient progress toward goals and update plan of care. Serve as a liaison and advocate for patients and families. Assist in managing transitions of care across care settings, ensuring optimal communication and planning. Identify barriers, facilitate solutions and connect others to community resources. Partner with other care coordinator teams such as primary and transitional care social work, rehabilitation, pharmacy, palliative care and others. Define and ensure compliance with disease-specific care paths for specialty care or chronic disease. Coach patient and family on self-management support. Educate about managing a specialty or surgical condition (inclusive of preoperative, perioperative, postoperative and recovery) inclusive of prevention and health maintenance tasks. Educate and connect to other care providers and community resources to enhance care. Minimum qualifications for the ideal future caregiver include: Graduate from an accredited school of Professional Nursing Current state licensure as a Registered Nurse (RN) Basic Life Support (BLS) certification through the American Heart Association (AHA) or American Red Cross Three to five years of nursing experience Preferred qualifications for the ideal future caregiver include: Bachelor’s of Science in Nursing (BSN) Specialty certification Outpatient and/or Emergency Department experience Advanced Cardiac Life Support (ACLS) through American Heart Association (AHA) Certification Care coordination experience Physical Requirements: Requires full range of motion, manual and finger dexterity and eye-hand coordination. Department/Unit: ICU-Surgical And Neuroscience - B2 Work Shift: Day (United States of America) Salary Range: $88,192.00 - $136,697.60 The Surgical and Neuroscience Intensive Care Unit (SICU and Neuro ICU) has a combined total of 30 intensive care beds designed to provide specialized care to the critically ill adult (14 and older) patient. The unit has a usual 50/50 split of patient population between the neuro and surgical patient population. The focus of care is includes patients with life-threatening conditions related to major elective and emergency surgical procedures, multi-trauma, traumatic brain injury, spinal injury, stroke, seizure, sepsis/multi-organ system failure, hemodynamic instability and failure to wean. The SICU and Neuro ICU patient needs encompass but are not limited to comprehensive physical assessments, intensive monitoring, treatment of laboratory values, ventilator management and hemodynamic monitoring. SICU-Neuro ICU nurses require the following skill set – ICP monitoring via EVD or bolt, spinal drains/catheters, brain oxygen monitoring, Artic Sun therapy, continuous EEG, intra-arterial catheter, non-invasive cardiac output, mechanical ventilation, continuous renal replacement therapy, reconstructive flap monitoring, and massive blood transfusion among others. Nursing will support the following on-unit procedures including placement of arterial lines, central lines, external ventricular drains and ICP monitors, SEPS drains, bedside bronchoscopy, abdominal washouts, tracheostomy, percutaneous endoscopic gastrostomy tube placement, lumbar punctures, chest tubes, pigtail drains and others. At least 2 years med/surg or progressive care experience required. The focus of care is includes patients with life-threatening conditions related to major elective and emergency surgical procedures, multi-trauma, traumatic brain injury, spinal injury, stroke, seizure, sepsis/multi-organ system failure, hemodynamic instability and failure to wean. The SICU and Neuro ICU patient needs encompass but are not limited to comprehensive physical assessments, intensive monitoring, treatment of laboratory values, ventilator management and hemodynamic monitoring. SICU-Neuro ICU nurses require or will develop the following skill set – ICP monitoring via EVD or bolt, spinal drains/catheters, brain oxygen monitoring, Artic Sun therapy, continuous EEG, intra-arterial catheter, non-invasive cardiac output, mechanical ventilation, continuous renal replacement therapy, reconstructive flap monitoring, and massive blood transfusion among others. Nursing will support the following on-unit procedures including placement of arterial lines, central lines, external ventricular drains and ICP monitors, SEPS drains, bedside bronchoscopy, abdominal washouts, tracheostomy, percutaneous endoscopic gastrostomy tube placement, lumbar punctures, chest tubes, pigtail drains and others. Job Description: • Implements physician orders, administers medications, starts IVs, performs treatments, procedures and special tests, and document treatment as required by company policy and local/state/federal rules and regulations. • Orders, interprets, and evaluates diagnostic tests to identify and assess patients' conditions. • Assesses and evaluates patient needs for, and responses to, care rendered. • Applies sound nursing judgment in patient care management decisions. • Provides primary and emergency care for occupational and non-occupational injuries and illnesses. • Administers over-the-counter and prescription medications as ordered. • Collaborates with the nursing team to create a Plan of Care for all patients. • Directs and guides ancillary personnel and maintain standards of professional nursing. Qualifications: • Graduate of a professional academic nursing program in which a Diploma, Associate Degree or Baccalaureate Degree is conferred • Must hold current NYS Registered Nurse license • Obtains and maintains certification in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) • Minimum of 1-3 years of previous clinical RN experience in Med-Surg or Progressive Care • Excellent communication, prioritization, organizational and time-management skills • Possesses knowledge of quality concepts, principles, and problem-solving tools and techniques and demonstrates ability to apply in support of departmental and patient care quality improvement.
