About the Unit: In this combined two units of 57 beds, you will take care of patients with critical heart conditions. You will monitor patients who have recently had a cardiac emergency or undergone heart surgery, such as a transplant or bypass. You will do everything from a STEMI to heart, kidney, & lung transplants. We offer a comprehensive orientation process, with highly supportive and experienced staff that provide education within the specialty. Our units are state-of-the-art equipped with the latest technologies in patient care. Our team conducts multidisciplinary rounds with the interdisciplinary team in identifying safe and efficient critical management high acuity acute care. What you will do this in this role: Apply nursing judgment through a thorough, systematic approach of applying the nursing process to include: Assessment: Conducts comprehensive, timely, and ongoing focused physical, psycho- social and pain assessments; includes assessment of patient education needs. Planning: Utilizes the assessment, identifies problems, makes nursing diagnoses, formulates goals, teaching plans and outcomes to create an individualized plan of care. Identifies individual patient goals, readiness to learn and incorporates family into the plan of care. Implementation: Prioritizes and implements a competent, prioritized and efficient nursing plan of care. Evaluation/Reassessment: Consistently conduct timely evaluation, anticipate and recognize changes in patient conditions, determines when reassessments are needed and adjusts the plan of care as necessary. Compare expected and achieved outcomes of nursing care utilizing the plan of care. Collaborate with patient care team members on patient care plan; communicate timely and effectively with physicians and other members of the patient care team through verbal, written or electronic communication to coordinate and facilitate meeting patient needs. Incorporate consistent use of hand-off communication and SBAR communication methods. Demonstrate critical thinking skills, scientific judgment, and organizational skills. Prioritize tasks and adjust priorities when necessary. Maintain an orderly, efficient and timely flow of patients. Make accurate, complete and timely entries into the patient medical record addressing all steps of the nursing process. Responsibilities Practices family centered nursing according to policies and Nurse Practice Act; serves as Charge Nurse when assigned; supervises, plans, evaluates, and coordinates patient care provided by other licensed and non-licensed staff; demonstrates CPR according to American Heart Association standards; abides by and enforces all compliance requirements and policies and performs these responsibilities in an ethical manner consistent with the organization’s values; adheres to hospital policies including confidentiality; serves as preceptor for new employees and nursing students; utilizes the nursing process to: assess patients; plan individual care; identify expected outcomes; implement and evaluate nursing interventions for patients and families; performs therapeutic procedures and assists with diagnostic procedures required for the care of the patient; administers medications including controlled substances by all routes; administers blood and blood products; responds appropriately to emergency patient situations; performs basic life support and management of arrest situations; starts IVs; performs activities of daily living; when performing skilled procedures the RN can state the purpose, rationale, precautions and observations related to that procedure; utilizes proper body mechanics when: moving, positioning and transferring patients without assistance or with assistance when needed for patient safety; moves equipment that is necessary to perform essential functions; provides patient education by planning, coordinating and delegating, as appropriate, the educational needs of each patient; utilizes a personal computer or terminal to access the Clinical Information System; communicates and uses appropriate customer relation skills with physicians, patients, families and healthcare team in person and via telephone; responds to overhead pages; completes ADP within unit time frame; completes flow chart, pain assessment tool, discharge forms, and MAR ; supervises, evaluates and finalizes the patient care record; transcribes physician orders; completes all mandatory unit, educational and hospital requirements; utilizes cost effective practices in performing all aspects of job; adheres to current Infection Control and Safety Standards; demonstrates the ability to provide competent patient care to the following patient populations: Neonate, Infant-Toddler/Preschool, School Age/ Adolescent, Reproductive Age, Post/Peri Menopausal; utilizes and cares for all types of CVL’s including PICC /Midlines; serves as PICC Line Team Member; provides on-call support to unit when Pediatric Critical Care Transport Team has been dispatched; maintains current knowledge base of all unit policies and procedures, equipment location and supplies; ensures that charge stickers and charges for all procedures are documented in the appropriate location; completes PALS and ACLS within one year of employment; maintains PALS and ACLS certification status with 6-month window to complete course; administers and monitors conscious sedation; identifies, organizes and directs activities to utilize resources, personnel, and materials to meet the needs of the patient and nursing staff; communicates with the Nurse Manager and Nurse Administrator, on an ongoing basis, regarding: changes in the unit status, patient related issues, Quality Management issues, and other events as they evolve; acts as first line in problem resolution with other departments and any complaints; assists with shift duty assignments; verifies that emergency equipment is functional; ensures that narcotic counts and stock drug counts are completed by end of shift; completes acuity sheet and charge nurse checkout sheet; maintains knowledge base of all PICU Critical Care Transport policies and procedures; maintains competency in all advanced nursing, transport procedures and skills; utilizes the appropriate communication skills with referring hospital personnel and transport team members throughout the transport process; participates on committees as assigned; participates in Performance Improvement activities as assigned; accesses and accurately maintains electronic and paper medical records; regular and prompt attendance; ability to work schedule as defined and overtime as required; related duties as required.
