Supports a smooth transition of patients from active treatment into survivorship, chronic cancer management, and end-of life care. Promotes advances care planning by assisting patients in formulating a discussion with their care team. Attend Tumor Board Conference and be available for coordination of care recommended post conference. Maintain communication with the tumor registry when needed. THE ROLE You’ll carry a full patient panel, managing perimenopause, menopause, weight, sexual health, mood, and sleep — all virtually. This is ongoing, relationship-based care, which means your patients come back to you — visit after visit, over months and years. You’ll prescribe and independently manage HRT (estrogen, progesterone, testosterone), evaluate patients for pharmacologic weight management including GLP-1 therapies, and make autonomous clinical decisions every day. It’s a high-volume environment. Documentation, follow-through, and productivity benchmarks are part of the job — and the NPs who thrive here don’t fight that reality, they’re built for it.
