Assists patients with cancer with issues related to treatment goals, advanced directives, palliative care, and end-of-life concerns using an ethical framework that is nonjudgmental and nondiscriminatory. Acts as a liaison between the patients, families, and caregivers and the providers to optimize outcomes. Advocates for patients to promote patient centered care that includes shared decision making and patient goals of care with optimal outcomes. Provides psychosocial support to and facilitates appropriate referrals for patients, families and caregivers. Contributes to ONN program and role development, implementation, and evaluation within the healthcare system and community. Participates in the tracking and monitoring of metrics and outcomes, in collaboration with administration, to document and evaluate outcomes of the navigation program. Job Description The Nemours Cardiac Stepdown Unit in Wilmington, DE has rotating shift availability for full time, every third weekend and every third holiday. Description The Nemours Cardiac Center (NCC) provides comprehensive care for infants, children, and adolescents with cardiac, thoracic, and vascular disease. The Cardiac Center provides patient care, performs research, and educates and trains health care professionals in various settings, including the Cardiac Intensive Care Unit and the Cardiac Step-Down Unit. Essential Responsibilities Assesses and documents in itial and ongoing patient needs and problems. Formulates nursing plan based on patient needs and problems and indicates plan on the patient care plan. Develops realistic and appropriate long- and short-term goals based patient/family needs.
