Summary The Registered Nurse is responsible for all departmental functions aligned with the mission, vision, and goals of PM Pediatrics. The Registered Nurse role is an opportunity for a pediatric nurse to apply their skills in a pediatric urgent care setting and assist with primary patient care based on individual patient needs within their scope of practice under the physician’s direct supervision. This position maintains effective working relationships with all customers, including patients/families, physicians, employees, volunteers, and vendors. Description This position requires the commitment of working every weekend (Saturday and Sunday). Candidates must be open and flexible to commuting to our sister office in Saugus as business needs require and at leadership's request. The Intake RN must answer calls entering the queue (or coming in on direct extensions) and process demographic, clinical and insurance information in a manner consistent with service excellence and that ensures efficient and effective call processing, leading to a patient admission to Pembroke Hospitals program or providing appropriate information to the caller. The Intake RN is an adjunct to the department and is available as an immediate resource to non- clinical staff to assist them in ensuring appropriate medical screening in order to facilitate timely and expedited placement of patients within Pembroke Hospital facilities who meet admission guidelines. Responsibilities Complete intake call sheet on all referrals including for inpatient as appropriate for the position. Gather all necessary demographic, clinical and financial information on call sheet. Ensure information is accurate and complete as call sheet is transmitted to hospital clinical and business office staff. Requests evaluation from ESP/ED/Referrer as appropriate. Assist Intake Coordinators when requested in interpretation of lab values, and diagnostic testing within Pembroke guidelines in order to facilitate appropriate referrals to facilities. Reviews for first call acceptance of appropriate patients based on Pembroke protocols. Notify the units of pending admissions. Send completed call sheets and insurance verification/authorization to Hospital Unit, Utilization Review and Business office. Maintain accurate, up-to-date bed availability including update hospital staff of pending admissions or cancellations. Update external bedboard on regular basis. Adhere to department procedures related to waitlisted patients, transferring cases. Contact the CEO when a review is necessary or patient delayed. Charge Nurse

Park Avenue Healthcare is looking for a Charge Nurse (7am-3pm) has 8-hour accountability. He/she is responsible for the overall operations, integration, coordination, and direction of nursing and patient care for the assigned unit. Additionally, he/she is responsible for ensuring that care delivery is consistent with the mission, vision, values, and policies of Best Care Services and in accordance with accepted standards of practice, state, and federal regulation and licensing requirements. This includes mentoring and education nurses, working with physicians and other medical professionals, and serving as a resource to all staff within the unit regarding the quality of services provided. RESPONSIBILITIES/ACCOUNTABILITIES: Collaborates with other departments to provide timely effective care consistent with individuals’ needs, choices, and preferences. Participates in meetings to address patient care, survey, and/or standards of care issues and utilization management customer at risk, care planning, quality improvement, etc. Participates with the interdisciplinary team in developing, implementing, and evaluating ways to achieve patient goals and minimize rehospitalizations. Ensures Point Click Care (PCC) is utilized according to the business processes. Participates in shift-to-shift communication between incoming and outgoing nursing staff and CNAs. Ensures discharge planning process I in place and provides consultation as needed. Ensures complete and prompt reporting of incidents, accidents, and adverse events to Center Executive Director and Director of Nursing. Ensures that patient’s accident/incident, adverse event, and grievances/concerns are fully documented, investigated, reported, and addressed in accordance with Revolution policies and procedures and the Federal/State rules and regulations. Quality Improvement: Participates in the development, implementation, and evaluation of quality improvement activities for the center and/or unit as requested. Prepares written reports, reviews records, and participates in needed data collections as indicated. Reviews clinical records of patients with unplanned rehospitalizations to identify areas in need of improvement. COMPLIANCE: Complies with and promotes adherence to applicable legal requirements, standards, policies and procedures including but not limited to those within the Compliance and Ethics Program, Standard/Code of Conduct, Federal False Claims Act and HIPAA.
