Charge Nurse 0.9 FTE, Nights, 1800 start, Monday through Wednesday As an integral member of the health care team, the Charge RN functions within the scope of practice as defined by the State and the ANA Scope and Standards for Pediatric Nursing Practice. Responsible for the planning, organization and delivery of safe and timely patient and family care for all patients on the unit during each shift or for providing safe direct patient care. Applies the knowledge and skill necessary to provide appropriate interactions with staff, patients and families of all ages. Applies the nursing process to the oversight and coordination of patient care for entire unit patient population during assigned shift. Assists in meeting Seattle Children's strategic plans by incorporating the initiatives to support the strategic plans into the daily work, i.e. Continuous Performance Improvement (CPI), ART, etc. In accordance with House Bill 1155 (effective January 1, 2020), the following areas have clinical unpredictable needs and as a result will have a call obligation within their nursing positions: Apheresis, Cardiac Cath Lab, Dialysis, ECMO, Home Care Services, Imaging Services (including GI Procedures), Perioperative Services, and Transplant Coordinators. Required Education and Experience Graduate of an accredited school of nursing; BSN preferred. Minimum one year nursing experience is required. Preference will be given to those with current experience in the area hiring e.g.: Peri Operative Services, PICU etc. and in pediatrics. Required Credentials Current Washington State RN License, or other current state license that the RN is required to work in. Current Basic Life Support (BLS) for Healthcare Providers is required for all positions. Pediatric Emergency Assessment, Recognition and Stabilization (PEARS), and Pediatric Advanced Life Support (PALS) will be accepted in lieu of BLS for Healthcare Providers. ACLS will not be accepted as a substitute to the requirements listed above. Minimum Qualifications: Two years of experience in home care, home health or related field. A reliable means of transportation with current insurance and a valid driver’s license. Must possess excellent written and verbal communication skills and be able to interact effectively with a diverse audience including clients, families, care providers and other professionals. Must possess impeccable follow up and attention to detail and be able to exercise initiative, problem solve, apply sound judgment. Able to supervise, assess and evaluate care providers and advocate on behalf of clients and families. Passionate about helping others live with dignity. Desired Qualifications: RN or LNP credential Five or more years in the in-home care industry Expert working knowledge of WellSky Previous small business experience Essential Duties and Responsibilities: Complete Client Assessment packet for each new client; provide a copy of all signed agreements to the client or designated client representative. Develop and implement a written care plan for each client with input and approval by the client and/or designated client representative. Each care plan shall include: Client’s functional limitations; Nutritional needs and food allergies for meal preparation; Home medical equipment and supplies relevant to the care plan; Type and schedule of services to be provided, including frequency; Non-medical tasks requested. Collect fees for initial services at the time of assessment or by the first day of service. Provide caregiver introduction for first scheduled shift; review care plan with client and caregiver and set expectations for service delivery; ensure client in-home binder with a printed copy of the care plan is easily accessible in the client’s residence. Supervise client care provided by agency personnel to ensure: Caregiver review the client’s care plan prior to service provision and whenever there is a change in the care plan. Caregivers comply with the care plan. Caregivers implement proper infection control practices. Caregivers observe, recognize and immediately report changes in the client’s service needs or condition. Caregivers are familiar with and implement emergency procedures Caregivers possess the necessary skills required for service needs All necessary supplies and equipment are available for on-site for safe client care Provided services fall within the scope of the agency’s license. Regularly communicate with the client and/or designated client representative to follow-up on service delivery and ensure client satisfaction. Perform scheduled and non-scheduled in-home client support visits. Conduct reassessments according to the agency’s established frequency and as needed due to a change in client condition or recent event. Participate in coordination of services when more than one community service is providing care to a client such as another home care agency, home health agency, hospice agency or facility. Serve as a resource for clients and families, facilitating client education, empowerment, and quality of life; provide recommendations and referrals as needed to third party services. Investigate issues related to client care and client or employee well-being. Document supervision of client care on the appropriate form and/or in the online management system in accordance with agency policies and procedures. Ensure client records are current and properly maintained in accordance with agency policies and procedures. Provide needed training and education to staff in the client’s home, via telephone and/or in the agency office. Registered nurses; child and adolescent psychiatrists; psychiatric nurse practitioners; psychologists; mental health therapists; social workers; pediatric mental health specialists (PMHS); special education teachers; occupational, recreational, art, music, and speech therapists; and students from these various disciplines all participate in the development of individualized treatment plans.
