Job Summary GENERAL SUMMARY We are seeking a highly skilled, compassionate Registered Nurse (RN) to deliver high-quality, patient-centered care through a combination of in-home patient visits, telehealth patient consults with Nurse Practitioners or Physicians as required, and administrative tasks. The CareConnect Team at WellBe supports timely response to patients’ changing needs and this role will be instrumental in delivering this care consistently and in a patient-first manner. This role primarily supports urgent/episodic interventions, working in partnership with physicians, nurse practitioners (APPs), and interdisciplinary care teams to address a range of patient needs across the care continuum. The RN will operate with a patient-first mindset, balancing strong clinical judgment with care coordination, education, and documentation responsibilities. The role has no direct reporting responsibility but will function as part of an integrated, interdisciplinary patient care team. CPR Certification is required. Prior Telehealth experience (preferred). Home care experience (preferred). Previous experience in hospice, palliative care, or geriatrics strongly preferred; other complex patient health care experience will be considered. Possess a valid driver's license with an acceptable driving record. Requires proof of an active auto insurance policy. Required Skills and Abilities: Ability to conduct in-home setting visits and have previous experience on a telehealth platform Demonstrated role proficiency with Microsoft Office; Excel, Word, G-suite, etc. Proficient with Microsoft 365 software suite, Electronic Health Records, Scheduling Software, and Ring Central applications Role model in courageous teamwork, integrity, ethics, and productivity Excellent verbal, written, presentation, and interpersonal communication skills Strong communication skills, with the ability to effectively interact with patients, families, and remote healthcare professionals. Ability to work independently and make sound decisions while adhering to established protocols and guidelines. Excellent organizational and time management skills to handle administrative tasks and facilitate telehealth consultations. Experience working with culturally diverse and underserved populations. Supervisory Responsibility : This position will not have supervisory responsibility. Travel requireme nts: Travel is required up to 100% locally to deliver patient care. Work Conditions: Ability to lift up to 20lbs. Moving lifting or transferring of patients may involve lifting of up to 50lbs as well as assist with weights of more than 100lbs. Ability to stand for extended periods. Ability to drive to patient locations (ie. home, hospital, SNF, etc) or community resource locations. Fine motor skills, and Visual acuity.
