Senior Registered Nurse - Hospital at Home

Location: Acute Care Hospital Tower Shift- 12hr Days (7am-7pm) Work Hours: This program is a 7 day a week service, rotating every other weekend and rotating holidays Employment Type: Full Time Hospital at Home is an innovative program that combines in-home with remote monitoring, offering patients timely, hospital-level care in their homes. Though the list of diagnoses enrolled in Hospital at Home continues to expand, common disease states include acute pneumonia, cellulitis, COVID-19, urinary tract infection, dehydration, or exacerbations of chronic conditions, such as heart failure or chronic obstructive pulmonary disease. For many patients, the program provides an alternative to the traditional hospital setting and results in improved patient outcomes and satisfaction. The Hospital at Home team is looking for motivated nurses with critical thinking skills, acute care experience, and an interest in serving the Parkland community. Primary Purpose The RN is a licensed professional who uses the Parkland nursing professional practice model to coordinate acute patient care delivery in the patient home. Using the nursing process, the RN assesses the patient in their environment, identifies nursing diagnoses based on responses to health problems, develops and implements an individualized plan of care, and evaluates the patient's response. Our phenomenal RN team also provides care for cardiovascular patients with conditions such Heart Failure, AMI, pre and post interventional patients (heart Cath, ablations, and pacemakers), patients needing cardiac drips, and Arrhythmia patients needing a higher level of care in a non-ICU setting. Location: Moody Outpatient Center 3rd Floor PRIMARY PURPOSE Responsible for the daily management of assigned unit to include staff supervision and development, planning, budget, quality, and facility operations and coordinating patient services to ensure high quality patient care and optimal outcomes. Directs staff meetings, interviews, trains, counsels and provides other personnel activities to ensure that clinical activities meet department and Parkland goals and mission objectives. Serves as a clinical resource, maintains skills and promotes staff development. Stays abreast of the latest developments, advancements and trends in the specialty area by attending seminars or workshops, reading professional journals, actively participating in professional organizations and maintaining Texas RN licensure. Collaborates with staff, physicians, administration and risk management to evaluate and improve patient services. Identifies and analyzes the design of jobs, work processes and flows in the unit and implements appropriate changes to improve effectiveness, productivity and efficiency that support the overall goals of the department and Parkland. Responsible for effective facility management as measured by cleanliness, proper type, amount and functioning of equipment and overall environmental safety. Oversees timely and effective maintenance of equipment and rooms. Responsible for space planning, including renovations, capital repairs and acquisitions. Ensures informed use of department resources through clear policies including equipment, supplies, personnel hiring/training practices and service utilization. Develops and administers the annual budget (capital, operating and payroll) to ensure that the unit has the necessary funds to carry out established goals and objectives. What does the Infusion Registered Nurse (RN) do? Under minimal supervision, the Registered Nurse provides professional nursing care for patients adhering to national and company standards and guidelines for specialty regulations. Demonstrates knowledge of physiological, psychological, and developmental and age specific differences in the patient population including late adolescence, adulthood, middle age, and the elderly. Collaborates with the interdisciplinary team to provide for assessment and therapeutic management of patient/family needs. Responsibilities The essential duties and responsibilities: Plan, implement, coordinate, evaluate and promote the continuity of care consistent with established standards and practices. Assesses patient/family needs and provides teaching and other instructional material. Prepares and administers therapies under the direction and supervision of a physician. Administers all medications/blood products using correct medication/blood product, dosage/amount, route, time and technique. Assures pre-authorization and appropriate consent forms are obtained prior to administration of therapies where applicable. Facilitates dose modification by assessing patient, assessing labs, consulting protocol or treatment plan, calculating dose, and consulting physician regarding dosage modifications, problems and symptom management Performs venipuncture, accesses implanted ports and maintains central lines catheter devices. Maintains accurate documentation of assigned patient care and treatments provided. Applies knowledge of reimbursement procedures and charge capture. Qualifications The ideal candidate for the Infusion Registered Nurse (RN) will have the following background and experience: Education/Training Level I Graduate from an accredited program for professional nursing education, BSN preferred. RN state license and current registration with the State Board of Nursing in practicing state. Current Basic Life Support (BLS) certification. Must maintain continuing education requirements.
