MDS Coordinator RN The Sans Souci Rehabilitation & Nursing Center is looking for a talented and hard-working MDS Coordinator to join our ever-growing team. Captures the RUG score which reflects the care and services provided Demonstrates an understanding of MDS requirements related to varied payers including Medicare, Managed Care and Medicaid Ensures timely electronic submission of all Minimum Data Sets to the state data base. Reviews state validation reports and ensures that appropriate follow-up action is taken Facilitates the Care Management Process engaging the resident, IDT and family in timely identification and resolution of barriers to discharge resulting in optimal resident outcomes and safe transition to the next care setting Directly educates or provides company resources to the IDT members to ensure they are knowledgeable of the RAI process. Provides an overview of the MDS Coordinator and Assessor role to new employees that are involved with the RAI process. Teach and train new or updated RAI or company processes to interdisciplinary team (IDT) members as needed Analyzes QI/QM data in conjunction with the Director of Nursing Services to identify trends on a monthly basis Responsible for timely and accurate completion of Utilization Review and Triple Check Serves on, participates in, and attends various other committees of the Facility/Community (e.g., Quality Assessment and Assurance) as required, and as directed by their supervisor and Administrator Qualifications : Registered Nurse with current, active license in state of practice. Responsibilities What You’ll Do Provides comprehensive, individualized patient care in alignment with professional nursing standards. Specializes in chemotherapy and/or radiation oncology procedures. Assesses, screens, evaluates, educates, and manages care for adult patients in Outpatient Oncology Services. Supports both clinical practice and office operations. Demonstrates flexibility and teamwork by rotating across Oncology Service Departments as needed. Maintains working knowledge of University Hospital, Joint Commission, and Department of Health regulations. Actively participates in evidence-based nursing practices. Contributes to quality assurance (QA) and performance improvement (PI) initiatives. Qualifications What You’ll Bring Graduate of an accredited School of Nursing. Overview About the Role The Clinic Nurse Manager is an experienced clinical leader responsible for managing assigned areas and multiple services within the assigned practice, supervising assigned patient care personnel during clinic operations. The Clinic Nurse Manager is responsible to ensure the safe and effective delivery of patient care and clinical outcomes, compliance with regulatory, administrative, and clinical standards, policies, and procedures. Responsibilities What You’ll Do Maintains nursing and clinical operations by initiating, coordinating, and enforcing program, operational and personnel policies and procedures within the assigned practice in the Ambulatory Care Center. “Owns” clinic operations and exercises broad oversight for clinic patient throughput, provider performance, and clinical and/or support services. Maintains clinic supply inventory by studying usage reports, identifying trends, anticipating needed supplies, approving requisition and cost allocations. Measures patient, physician and employee satisfaction and develops and implements plans to improve satisfaction in collaboration with the team. Qualifications What You’ll Bring Required: Licensed as a Registered Nurse in the State of New Jersey. Bachelor’s Degree in Nursing (BSN) required, a Master’s Degree (MSN) preferred. A minimum of five (5) years of nursing experience required. BLS (CPR) is required. Certification as an Ambulatory Nurse (RN-BC) is desirable and preferred within two (2) years of hire.
