Under the supervision of the General Manager and/or Director of Clinical Services, the Registered Nurse or RN Case Manager is responsible for assessing & monitoring patients. RN performs initial patient assessments & home safety evaluations, designs, enters & implements the plans of care, and conducts periodic re-evaluations (every 3-6 mos).
