Job Description Responsible for working with other healthcare professionals to help treat patients with various injuries, illnesses or disabilities. Monitors patients, administers medications, keeps records, consults with healthcare providers, educates patients and more. Principal Accountabilities Maintains accurate, complete health care records and reports. Administers medications to patients and monitors them for side effects and reactions. Prescribes assistive medical devices and related treatments. Monitors, reports, and records symptoms or changes in patient conditions. We're looking for a motivated, team-oriented MDS Coordinator . The right candidate must have good communication skills in order to work effectively with the Interdiciplinary Team and keep acurate, detailed records. Familiarity with Point Click Care (PCC) and 1-2 years of recent MDS experience preferred.
