Overview Job Duties Under the supervision of the Ambulatory Administrator or Nurse Manager, directs, coordinates, and controls the nursing activities of his/her assigned staff. Helps set up telemedicine visits and patient education. • Accurately and efficiently documents findings, plans and interventions in the electronic medical record. Evaluates the effectiveness of care and documents subsequent changes to plan in the patient medical record. • Attempts to reduce readmission rate by counseling patient over phone and bringing patient in for in-person visit with Provider. • Counsels CHF patients and ACS patients on disease process. • Prescription refills (Cardiac patients on a great deal of medications). • Responds to all Patient Access Center task, patients issues. Triage medical issues. • Responds to physicians’ task in EPIC. • Review all lab work, screening of lab work. Makes MD aware of abnormal test results • Works with pharmacist on medication issues. Identifies discrepancies and duplication of medication. • Peer to Peer counsel with Insurance companies. Coronary Care Unit (CCU) Intermediate Nurse RN 7A-7P

Overview The UF Health Cardiac Critical Care Service Line includes a 17-bed Cardiovascular ICU (CVICU) and a 24-bed Coronary Care Unit (CCU) , united under one leadership team to provide seamless, high-quality care for adult cardiac medicine and cardiac surgery patients from admission through discharge. The CVICU delivers high-acuity critical care, including advanced life-support therapies such as invasive hemodynamic monitoring, mechanical circulatory support (Impella & Intra Aortic Balloon Pump), CRRT, temporary pacing, advanced airway management, and ECMO, while the CCU manages intermediate-level patients from the same population. Patients include those with acute coronary syndromes, heart failure, arrhythmias, cardiogenic shock, post–cardiac arrest, and those recovering from CABG, valve and structural heart procedures (TAVR), PCI, ablations, and pacemaker placement. Nurses are cross-trained in cardiac surgery and cardiac medicine care and supported by a structured professional development pathway that allows progression from intermediate to advanced open-heart and critical care roles. Care is delivered in all-private rooms with 4:1 CCU and 2:1 CVICU nurse-patient ratios, supported by an engaged charge nurse and a multidisciplinary team of physicians, advanced practice providers, pharmacists, therapists, and specialized nursing roles, fostering collaboration, professional growth, and excellent patient outcomes. MDS COORDINATOR- Exempt (Salary) Position Reports to Administrator OVERVIEW: MDS Coordinator administers patient assessments and overseas the assessment process, setting the assessment schedules and assuring that assessments are done in an accurate and timely manner. The MDS Coordinator coordinates the care plan according to regulatory requirements. Ensures that resources are made available to patients and patient care is delivered effectively and to a satisfactory standard. Creates the schedule for all Medicare and Medicaid. Start Medicare coverage for newly qualified patients and remain updated on changes in Medicare coverage and help determine documents needed for reimbursement. Acts as an in-house Case Manager demonstrating detailed knowledge of residents' health status, critical thinking skills to develop an appropriate care pathway and timely communication of needed information to the resident, family, other health care professionals and third- party payers. Proactively communicates with Administrator and Dircetor of Nursing to identify regulatory risk that allow capture of resources provided on the MDS, and clinical trends that impact resident care. Reviews state validation reports and ensures that the appropriate follow up action is taken.
