DEPARTMENT: Nursing REPORTS TO: Principal/Nursing Supervisor SALARY: RN Substitute salary schedule WORK SCHEDULE: To be arranged with the Principal and the Nursing Supervisor of Nursing (In accordance with contract requirements). DUTIES: 1. Assess and evaluates students health pertinent family history and home factors, counsel students, parents and school personnel about health problems and issues. 2. Maintain a health records, conduct, require research and evaluate school health programs. 3. Notify appropriate individuals of immediate and potential health related problems. 4. Coordinate health resources of school, home, community and allied health agencies/hospitals by making necessary arrangements, maintaining records, notifying and counseling students, parents/guardians and health personnel 5. Prepares regular written reports for supervisor, MDPH and other relevant agencies 6. Develops an individual health care plan as needed and coordinates with the other providers and school personnel 7. Documents pertinent information in student records according to the Standards of Practice 8. Provides emergency care for all students on school premises in the event of injury or sudden illness 9. Medication administration to students with parent/guardian and physician consent 10. Perform site mandated screenings in vision, hearing, scoliosis, TB testing and other health related activities as deemed appropriate by state and city departments of public health 11. Provide information about health and disease prevention in cooperation with health education programs as requested by administrators 12. Participates in school health related activities as deemed appropriate for school nurse by principal/supervisor 13. Acts as a resource in health education to students and families 14. Initiate, promote, maintain and implement programs for the recognition, prevent and control of disease, including giving immunizations. 15. Collaborate with staff in school and health centers. 16. Sport Physical clearance coverage as needed. QUALIFICATIONS: Must have a current valid, license to practice as a Nurse in Massachusetts. Have three years of responsible professional experience in school nursing or a related field preferred. Registered Nurse (RN) Job Responsibilities: Accountable for planning, implementing, evaluating, and communicating all phases of nursing care for assigned patients in our Psychiatric Inpatient Program. Assesses the health status of patients/families by means of comprehensive health history and physical examination. Initiates selected therapeutic and diagnostic procedures, consultations, and laboratory examination, interprets findings, and alters plan of care according to collaborative practice agreement and protocols in designated area. Educates and counsels in the areas of health promotion, maintenance, disease prevention, and management of acute/chronic illnesses. Documents findings and plan of care in the patient's medical record. Consults with physician per established Day Kimball Hospital policy. Intervenes to provide a safe environment and evaluates effectiveness of intervention for patients. Registered Nurse (RN) Education and Experience Requirements: Graduate of an accredited school of nursing is required. Registered Nurse (RN) license issued by the state of Connecticut is required. American Heart Association Healthcare Provider CPR certification or equivalent is required; if not currently certified, must obtain within three (3) months of employment. Additional specialty certification/training as required by the work area. Current NVCIT (Non-Violent Crisis Intervention Training) certification or within 3 months of hire required. Registered Nurse (RN) Knowledge, Skills and Abilities: Trained and competent in responding to events where a patient is out of control or exhibiting inappropriate behavior; ability to de-escalate volatile situations. Demonstrated level of psychological and emotional tolerance and an even temperament in crisis situations. Knowledge in Psychiatric Nursing. Demonstrated skills in critical thinking, effective communication, organizational skills, problem solving, and ability to make appropriate independent decisions. Working knowledge of applicable Standards of Practice. Demonstrated compassionate customer service and ability to meet the needs and expectations of patients and health care colleagues. Summary: The Registered Nurse (RN) plays a critical role in executing InterCommunity mission to promote whole person health. Under the supervision and direction of the Clinical Practice Manager, the Registered Nurse is responsible for providing services which consists of an evaluation and assessment of medical and psychiatric status as an integral part of the interdisciplinary team. The Registered Nurse also provides direct patient care in the departments of primary care and psychiatry. Essential Duties & Responsibilities: Provides direct patient care, carrying out orders given by my medical provider, which may include obtaining medical histories, obtaining vital signs, performing office lab testing and phlebotomy/ venipuncture, EKGs, wound care, and which includes but is not limited to medication administration. Conducting nursing visits or educational groups as ordered by the medical provider Maintains accurate, detailed reports and records. Records and reconciles patients' medical information. Monitors, records, and reports symptoms or changes in patients' conditions, assess efficacy of prescribed medications. Ensure gaps in care for screenings, chronic care tests (such as HgbA1c for diabetes), BMI, vaccinations, are completed in timelines established by best practice. Facilitates effective crisis intervention. Perform safety assessments and triage for patients in crisis. Consults and coordinates with healthcare team members to assess, plan, implement, or evaluate patient care plans. Modifies patient treatment plans as indicated by patients' responses and conditions and collaborate within a multidisciplinary team to achieve best patient outcomes Maintains an active 10- patient panel for Complex Care Management (CCM). Performs patient triage of acute conditions in-person or over the phone. Refers patients to specialized health resources or community agencies furnishing assistance. Performs other duties as required. *All agency staff are required to attend all mandatory department/agency meetings and trainings Requirements Education &/Or Experience: Graduation from an accredited nursing program. Current State of Connecticut RN licensure required. One (1) year medical experience in a public health setting required. Current Connecticut RN License required. 2 years of community health, addiction medicine and/or psychiatric nursing required. Basic Life Support (BLS)/ CPR certification. EPIC experience preferred, but not required. Bilingual (Spanish Speaking) preferred, but not required.
