Overview The Oncology Nurse Navigator (ONN) is the main point of contact for an assigned group of cancer patients, their families, caregivers, and referring physicians. The ONN demonstrates excellent critical thinking skills and works collaboratively with a multidisciplinary team of physicians, allied health providers, and administrative support staff to identify and overcome barriers to care, ensure care is seamless, provide timely access to care, and provide education and support throughout the patient's cancer continuum. The ONN organizes and facilitates weekly multidisciplinary tumor conferences for the assigned disease site group(s) to ensure case materials such as radiology, pathology, and outside treatment notes are assembled before the conference. Additionally, the ONN is responsible for ensuring that patients are scheduled appropriately, recommended patient follow-up occurs, and patients are educated on their diagnosis and conference recommendations. The ONN serves as the liaison between internal and external referring physicians, other members of the care team, and patients to ensure clinical and administrative information is communicated timely, patients' emotional and support needs are addressed, and the patient has a seamless transition of care throughout the UF Health system. The ONN will collaborate with the department manager, physician leaders, and the Cancer Centers Community Outreach and Engagement team to participate in community outreach and education for their assigned disease site group(s). Overview Registered Nurse Cardiac Device Specialist will focus on pacemaker/ICD wound checks and follow-ups, reviewing remote monitoring transmissions, provide triage of symptoms for events, and documentation in EMR systems. Will also see patients for wound checks and device placement follow-up visits. They will possess specialized knowledge and skills to provide patient-centric care to patients in a physician's office setting.
