This RN, consistently provides effective direct care to patients in their homes for an assigned caseload of patients; works as part of the interdisciplinary team; oversees the care planning and care coordination for a variety of complex patients. Manages care and implements treatment plans at a refined skill level in collaboration with patients and their families. This RN works directly with Pharmacists, Physicians and other members of the health care team and ensures compliance with homecare standards and regulatory requirements. KEY RESPONSIBLITIES Administer infusion therapies including, but not limited to, antibiotics, biologics, hydration, parenteral nutrition, and other specialty medications. Provide patient and caregiver education on medication administration, infusion equipment, and infection prevention. Assess and monitor vascular access devices including PICC lines, midlines, and central venous catheters. Perform dressing changes, lab draws, IV line placement, and medication management as ordered. Monitor patient response to therapy and communicate clinical updates to physicians and pharmacists. Coordinate care with hospital discharge planners, outpatient providers, and pharmacy teams. Document assessments, treatments, and patient education in the electronic medical record. Participate in an on-call rotation to support urgent patient needs.
