Enliven is seeking an experienced Registered Nurse to service home infusion and Infusion Center patients in Tyler, TX, and surrounding locations. The ideal candidate must be well-trained and able to give the best nursing care possible with little supervision. They will educate patients and their families on prevention and healthy habits. The ideal candidate must be compassionate, cool-tempered, and capable of handling emergent situations. They must follow health and safety guidelines faithfully and consistently. The goal is to promote patient's well-being by providing high-quality nursing care. Responsibilities: Complete daily electronic forms for each assigned patient within a secured Electronic Medical Record (EMR) System. Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients. Administer ongoing care/infusions for each patient, and provide necessary follow-up as directed by the Physician and Clinical Manager. Confer with physicians and pharmacies in developing the initial plan of treatment based on the physician's orders and initial patient assessment. Provide hands-on care, management, and evaluation of the care plan and teaching of the patient under a physician based on ongoing assessments and as required by policy/regulation. Report patient care/conditions/progress to the patient's physician/pharmacy and Clinical Manager continuously.
