Overview: The Transportation RN (0.6 or 0.8 FTE) is a key member of the School-Based Health Transportation Team, coordinating care for school‑aged children who receive school‑district–provided transportation between their home school and the Nationwide Children’s Hospital School-Based Health Clinic (SBHC). The SBHCs offer a variety of primary care–level services, and the RN supports access to this care through telephone outreach to parents or legal guardians to review health histories, obtain informed consent, and offer services. The RN ensures that accurate, complete clinical information is documented and communicated to the SBHC provider to support safe, seamless care. This position follows the school-year calendar and includes summers off. When assigned by a Special Education Case Manager, design appropriate goals, objectives and timelines for appropriate school related special education health services for eligible students. Provide quality educational based special education health service based on goals related to deficit areas. Monitor and document student progress and communicate progress with appropriate students, staff, parents and other members of the school-based team(s). Develop care plans for and serve as a resource person in the educational planning for medically fragile students, as requested. Act as a liaison between home, school and community resources and makes referrals to appropriate community agencies. Provide health counseling to students/staff regarding chronic illness, nutrition programs and positive lifestyles as appropriate. Perform various types of health screening assessment activities and cooperates with other district screenings, make recommendations, and do follow-up based on the results of these screenings. Follow time schedules, attends and participates in all assigned meetings, and maintains a positive professional manner with students, parents and staff. Analyzes the total health care of students with chronic illnesses/disorders, and performs designated routine non-invasive and invasive procedures as established by physician orders. Attends to any students health needs as indicated in the School Health Services Manual. Essential Functions 1. Performs routine and prescribed invasive and non-invasive procedures including, but not limited to; gastric tube feedings; breathing treatments; diabetic glucose monitoring; urinary catheterizations; and tracheostomy care and suctioning. 2. Identifies and monitors student health status through needs assessments, screening procedures, examinations, observations, and analyses. 3. Establishes and maintains appropriate medical records, logs, indices, and schedules on daily and school year diagnoses, care plans, procedures, treatments, monitoring, adverse reactions, and notifications. 4. Provides basic first aid and attends to student health needs as indicated in the School Health Services Manual. 5. Distributes/administers prescription and non-prescription medication to students in accordance with applicable laws and directives, following guidelines described in school health policy manual. 6. Assists the student, school personnel, and parents in understanding and coping with a student's disability or condition. 7. Designs intervention in conjunction with other health and education team members to prevent further disability. 8. Facilitate Telehealth appointments for students. 9. JOB DESCRIPTION PERRIS SCHOOL DISTRICT SCHOOL NURSE Definition: Under the direction of the Assistant Superintendent of Educational Services, the school nurse develops and maintains a broad program of health education and services designed to protect, maintain and enhance the health of students. The school nurse strengthens and facilitates the educational process by identifying and remediating health related barriers that may impede the ability of students to learn. Responsibilities: 1. Conduct mandated health screenings, i.e., vision, hearing, and follow-up; complete related required annual reports and maintain health/records. 2. Monitor the safe administration of medication for students with physician order to take prescription medication at school. 3. Participate in the identification and evaluation of students recommended for special education assessment; complete developmental history reports and participate in IEP meetings when appropriate. 4. Monitor compliance with immunization requirements; complete required immunization reports, interpret immunization requirements and rationale to staff, students, and parents; assist parents in complying with District immunization requirements. 5. Provide counseling and assistance to parents and staff for referral and placement to appropriate program and services regarding health related issues. 6. Serve as a health education and staff development consultant to classroom teachers and parents. 7. Assist in the implementation of the District health curriculum, particularly in the areas of drug, alcohol and tobacco use prevention education, family life education, AIDS education and control of communicable diseases. 8. Teach, supervise, mentor, and evaluate Specialized Physical Health Care Service (SPHCS) procedures provided to identified students. 9.
