The Licensed Practical Nurse (LPN) , under the supervision of the Clinical Supervisor, follows established policies and procedures of UChicago Medicine Home Care and the Nurse Practice Act. Performs total direct and indirect skilled nursing care under the supervision of a Registered Nurse. Acts independently to assess the nursing needs of the patient and works either singularly or in conjunction with interdepartmental staff to meet the individual needs of the patient. Essential Job Functions Makes home visits to provide direct patient care within the scope of practice as assigned by registered nurse. Observes patient progress or lack of progress and communicates these observations to the registered nurse for adjustment of the patient’s plan of care. Documents observations and nursing interventions including medication and treatment according to the patient’s plan of care and agency policy and procedures. Maintains ongoing communication with the registered nurse regarding all aspects of patient care interventions, needs and changes. Teaches the patient’s family to carry out preventative and rehabilitative measure, in an effort to increase their responsibility of independence or toward independent living. The Home Infusion Nurse provides intravenous medication administration for patients in the home and the infusion suite setting. The professional nurse plans, coordinates, and performs home infusion services with emphasis on community health education/experience. The professional nurse works cooperatively with pharmacy staff to establish priorities and goals of patient care. Essential Job Functions Administers medications and treatments as prescribed by the physician. Monitors patient response to interventions. When providing home infusion skilled nursing services, they will assess and monitor patients for anaphylaxis and intervene as needed. Successfully instructs patients on home infusion therapy self- administration, provide direct nursing care in patient’s home for the administration of home infusion therapies, and troubleshoot patient’s infusion concerns. Regularly re-evaluates patient nursing needs. Administers medications and treatments as prescribed by the physician. Monitors patient response to interventions. Includes the patient and the family in the planning process. Complete all required patient care documentation, nursing plan of care and reports. Other Duties as Assigned Position Qualifications One to two years of acute care experience in an institutional setting. Possesses and maintains a valid driver’s license and current automobile insurance and has the availability of personal and dependable mode(s) of transportation to conduct home visits. Current state of Illinois nursing license BLS ACLS Chemotherapy & Immunotherapy Class – Adult Hematology Oncology Bridging Course; Fundamentals ONS Chemotherapy/ Immunotherapy Certificate within 12 months of hire.
