The Residential Program Nurse provides training, supervision, and oversight for both residents and staff in all aspects of medical and medication management. They will collaborate with the Nursing Director, Residential Program Directors and nursing team in the implementation and follow-through of the individualized service plans and documentation in accordance with the mission and regulations of the program. Develop an individualized Emergency Contact sheet, which includes family, guardian, physicians’ (PCP and specialists), clinic/hospital contact information, Social Security Number, MassHealth ID number, date of birth, gender, guardianship, health care proxy, marital status and code status, and program start date for each new resident prior to his/her start at the program. Assist residents with all aspects of medical management including physicals, referrals, prescriptions, appointments, and treatments. Coordinate with residents, family members, physicians, other medical providers, and site team regarding all medical care and changes in status, treatment, or medication. Implement, oversee, and supervise medication management and best practices in accordance with agency policy relating to MAP in a manner consistent with funder regulations.
