Job Overview The Clinical Care Technician works under the supervision of a professional nurse and collaborates to meet the needs of the patient within the scope of the role. Assists with patient care procedures as directed by the nurse, using proper body mechanics and ensuring patient dignity. 3. Greets patients, families, visitors and other staff according to service standards. Responds to patient and family requests promptly, seeking nurse’s supervision as needed. 4. Collects, records, and communicates the patient’s clinical data to the assigned nurse in a timely manner. 5. Prepares room for admission/transfer/discharge, assuring standardized equipment is ready for use. 6. Transports or accompanies patients as necessary to other areas of the hospital in an efficient, safe, and respectful manner. Acts as a sitter as requested by the nurse, adhering to the sitter guidelines, to ensure patient safety at all times. 7. Provides comfort measures such as positioning, changing linen, back rubs, etc. to patients in an efficient, safe, and respectful manner. 8. Promotes patient activity by assisting with mobility and range of motion exercises as assigned 9. Sets up for procedures as requested by nursing/medical staff. 10. Cleans and discards used equipment and supplies following procedures. 11. Collects specimens and performs specimen testing, while maintaining Standard Precautions; prepares specimens for the lab. 12. Responsible for care of biomedical equipment, checking par levels and working order for blood pressure cuffs, EKG cables, leads, oximeter cables, etc. Sends malfunctioning equipment to Medical Engineering per established procedure. 13. Cleans wounds and applies DSD after wound is assessed by nurse. 14. Performs 12 lead EKG. 15. Connects monitoring equipment to patients under the direction of the nurse. 15. Cares for patient belongings and completes belongings list. 16. Primes intravenous tubing with solution containing no medication. 18. Performs and documents patient’s admission, transfer, and discharge tasks as requested by the nurse. 19. Answers call lights and seeks appropriate personnel/action. Performs hourly rounding per established guidelines. Physical Requirements: 1. Lifting up to 30-35 lbs and turning patients. Prolonged standing and walking. Occasional twisting, bending, reaching, pushing/pulling, sitting, kneeling, and squatting. 2. Manual dexterity using fine hand manipulation for performing procedures to operate computer keyboard. 3. JOB DESCRIPTION Job Summary Provides support for care management/care coordination long-term services and supports (LTSS)-specific activities. Job Description: I. Position Summary: The Licensed Practical Nurse (LPN), in collaboration with a Registered Nurse, utilizes the nursing process of assessment, planning, implementation, and evaluation to ensure quality nursing care and to maximize the quality of life for the patients. The LPN is an integral member of the clinical support team, and collaborates with other multidisciplinary members to meet the clinical needs of the patients. Position Responsibilities: Ensures all care delivered is within the scope and established standards of safe professional nursing practice with core care competencies Collaborates with other members of the multidisciplinary team (MD, NP, nurses, social worker, therapists, dietician) to meet the clinical needs of the patient and monitor outcomes of care Adheres to best practice recommendations and implements evidence-based interventions to ensure optimum outcomes for patients Practices in a primary nursing model and maintains accountability for a set of assigned patients with RN oversight of assessment and care planning Collaborates and participates in care planning activities and supports autonomy and individual choice wherever possible In collaboration with the RN, ensures the plan of care is an accurate reflection of the patient’s individualized needs and ensures that the plan of care is followed and revised as necessary. Documents pertinent information regarding care delivered in designated areas of patient’s record using the electronic medical records. Participates in the collection and monitoring of quality data to ensure high standards of care Participates in performance improvement projects Participate and assists with unit activities as needed (i.e. assist with serving meals) Serves as a coach and mentor to patient care associates May be assigned charge nurse responsibilities with RN oversight. Responsibilities may include: Giving shift report to on coming staff Provide clinical/supervisory oversight to the unit during a shift Delegate assignments to team assigned to shift Audit/assure completion of admission or discharges during shift with the RN’s cos-sign Attend rounds (wound, physician etc) If acting in the capacity of charge nurse, may develop patient care assignments (appointing other staff, RN, LPN or PCA to implement an established care plan for a designated period of work time), create schedules, and contribute to performance evaluations and educating other staff on HSL policy and procedure. The above covers the most significant responsibilities of the position. It does not, however, exclude other duties, which would be in conformity with the level of the position. Qualifications Graduate of approved school of nursing required Current Massachusetts license as an LPN in good standing Previous experience preferred Understand the philosophy and support the model of care at HRC required Must be professional, proactive, collaborative, conscientious and results-oriented individual.
