The Undiagnosed Mass Clinic (UDM) PA-C/ARNP is a physician assistant or advanced registered nurse practitioner who provides dedicated support to this growing Fred Hutch program, which serves as a diagnostic entry point for patients with suspected cancer. The APP provides comprehensive evaluation, diagnostic workup coordination, and medical management for patients referred to suspected malignancy, unexplained mass, or abnormal diagnostic finding requiring further oncologic assessment. The APP independently evaluates new referrals, develops diagnostic plans, orders and interprets testing within scope of practice, coordinates care across specialties, communicates findings to patients and referring providers, and facilitates timely transition to disease-specific oncology teams when cancer is confirmed. When cancer is not identified, the APP collaborates with referring providers and primary care teams to ensure appropriate follow-up and disposition. The UDM APP can practice at the intersection of oncology, diagnostic medicine, and care coordination. Significant collaboration with nurse navigation, pathology, radiology, and disease-specific oncology teams is necessary. All clinical responsibilities are performed within the APP scope of practice, institutional privileges, and under the supervision of an attending physician. This is a part time (50% FTE) position. Responsibilities Perform comprehensive and problem-focused H&P for patients referred to the UDM Clinic. Review referral records, imaging, pathology, lab studies, and prior evaluations. Develop, order, and manage diagnostic workups for patients with suspected malignancy. Interpret and communicate diagnostic findings to patients, referring providers, and consulting teams. Coordinate diagnostic testing, specialty consultations, and follow-up evaluations across multiple disciplines and healthcare settings. Collaborate closely with wide range of team members and specialists to ensure efficient progression through the diagnostic process. Track and manage pending studies, referrals, pathology results, and other diagnostic milestones to ensure timely completion of evaluations. Provide follow-up care and medical management while diagnostic workup is underway. Facilitate hand-offs and referrals to disease-specific oncology services when cancer is diagnosed. Coordinate care transitions back to primary care providers, referring physicians, or other specialty services when malignancy is excluded. Provide patient and caregiver education regarding diagnostic plans, test results, treatment recommendations, and follow-up care. Serve as a clinical resource for nurse navigation and intake referral processes. Participate in multidisciplinary conferences, QI initiatives, and departmental committees as assigned. Communicate effectively with inpatient and outpatient teams to support continuity of care. Participate in program development efforts that support growth, operational efficiency, and access within the UDM Clinic. Additional duties as assigned. Qualifications Required: Graduate of an accredited Physician Assistant program or Nurse Practitioner program. Current Washington State licensure as a Physician Assistant-Certified (PA-C) or Advanced Registered Nurse Practitioner (ARNP), or ability to obtain licensure prior to employment. DEA registration required upon credentialing. Minimum of four years of APP experience in oncology, hematology, surgical oncology, hospital medicine, internal medicine, or other relevant specialty. Demonstrated clinical expertise in complex patient evaluation and diagnostic reasoning. Strong collaborator with the ability to work effectively independently and with a multidisciplinary team. Excellent verbal and written communication skills. Experience utilizing electronic medical record (EMR). Strong organizational skills and ability to manage multiple patient workups simultaneously. Ability to work independently while maintaining collaboration with supervising physicians and teams. Preferred: Broad experience in oncology, cancer diagnostics, hematology, and complex medical evaluation. Experience coordinating multidisciplinary care across healthcare systems. Familiarity with cancer screening, diagnostic imaging, pathology review, and oncologic referral pathways. Familiarity using NCCN and other professional guidelines to direct care. Experience with quality improvement and program development initiatives. The Transition Registered Nurse provides transition and discharge teaching/planning for patients along the continuum of care. Care planning includes assessment, evaluation and integration of care with a multi-disciplinary team, focusing on continuity of care throughout the system. The Clinical Nurse Coordinator provides clinical support to medical providers, providing care management to their entire panel of patients. As the primary point of contact for complex outpatients, the Heme CNC provides continuity of care through nursing assessments, triage, patient education, and care coordination. The clinical nurse coordinator is an integral part of a dedicated multidisciplinary team that collaborates closely to provide the highest level of care for patients. This is a per-diem position at our South Lake Union location. The schedule for this position is Monday through Friday from 8:00 am to 4:30 pm. Responsibilities Assesses physical, emotional, social, and spiritual needs and evaluates patients’ adaptation to health changes Develops a plan of care based on patients’ disease, symptoms, and response to treatment Educates patients, families, and caregivers on disease processes, treatment, side effects and symptom management Coordinates with inter-disciplinary team to ensure continuity of care Refers patients, families and caregivers as appropriate to other clinical resources/professionals such as social work, home health care, clinical nutrition, etc. Implements, evaluates and modifies plan of care Integrates research and evidence based knowledge into clinical practice Care Coordination: Performs comprehensive initial assessment of patient, family and caregiver Performs ongoing assessment of patient, family and caregiver as patient progresses through Transplant process Monitors patient response to therapies, including lab values and symptom assessment, in collaboration with the medical, nutrition and pharmacy staff Provides psychosocial support to patients and families Functions as resource for rotating physician staff regarding standard practice and operations in the clinic Reviews all orders written by the medical staff Communicates hospital admissions to the Charge RN Coordinate home infusions provided by outside Home Infusion agencies after initial referral by Transition RNs Provide continuity of nursing care by communicating effectively with Charge, Evening, Weekend, Infusion Room, Procedure, Triage, and Inpatient RN’s Documents all assessments, teaching, response to therapies and plans of care in a timely manner Keeps patient Kardex up to date Functions as primary telephone contact for patients and their caregivers Participates with all disciplines in daily rounds to plan and coordinate patient care Patient and Caregiver Teaching: Provides initial education and Clinic orientation. Reviews Patient Resource Manual with patient and family at arrival visit Assumes primary responsibility for chemotherapy teaching with patients and caregivers Assumes primary responsibility for patient education regarding medications and therapies, including home infusions Assumes primary responsibility for patient education regarding symptom management Assumes primary responsibility for departure teaching when patient is leaving the Acute Transplant Program Support for Research: Acts as patient advocate in coordination of research consent forms Assists in obtaining research data in conjunction with patient care data Maintains a working knowledge of current protocols related to the patient population with whom they work. Knows research resources and how to access Communication and Teamwork: Participates in Committees and Task Forces to review and improve current standards Serves as a resource to other nurses caring for patients undergoing BMT transplant Flexible in incorporating patient care outside own team assignment to assist with patient care needs in other areas of clinic Informs Nurse Manager of high-risk patient situations or operational issues Communicates in a respectful and effective manner with other staff Demonstrates appropriate problem solving when working with other departments Maintaining And Developing Own Skills: Participates in continuing education programs Keeps current in area of expertise through journals, books, and conferences Regular attendance and punctuality are essential functions of this job in order to carry out the primary responsibilities. Qualifications Required: Must have current Washington State Registered Nurse (RN) license or current NLC multistate license (MSL); and MSL primary state of residence must be updated to WA within 60 days of hire. This requirement may be waived under specific circumstances, subject to approval. Applicants that do not have previous oncology experience will be provided additional training and support. Current BLS AHA Health Provider Card (or equivalent course, such as by the American Red Cross) and renewal required every two years. Additional certification may be required dependent upon department. Critical thinking; the ability to practice the cognitive skills of analyzing, applying standards, discriminating, information seeking, logical reasoning, predicting, and transforming knowledge. Ability to develop clinical judgment. Time Management skills; the ability to organize and manage time and tasks independently. Ability to communicate effectively by looking and listening for cues, asking open-ended questions, exploring cues, using pauses, screening responses, and clarifying responses. Ability to effectively listen by using reflecting, acknowledging, summarizing, empathizing, and paraphrasing skills. Ability to effectively provide information by checking what information the person knows already, giving small amounts of information at a time, using clear terms and avoiding jargon, avoiding detail unless it is requested, checking for understanding, and pausing and waiting for a response. Preferred: Bachelor of Science in Nursing. Two years of nursing experience. The hourly pay range for this position is from $54.78 to $101.42, which includes the per diem differential, and pay offered will be based on experience and qualifications.
