Graduation from an accredited school of licensed vocational nursing or psychiatric technology. Associate degree preferred. Experience: Minimum of one (1) year of experience with medication administration (Required). One (1) year of experience treating individuals with mental illness in an inpatient setting (Preferred). Safety & Compliance: Must be 18 years of age or older. Current CPR certification. First Aid certification from an approved provider (such as the American Red Cross). How you will make a difference : The LVN / LPT is responsible for providing direct client care, including medication administration and supervising the nursing activities of unit staff. Key duties include ordering, preparing, and documenting medications according to physician orders and confirming pharmacy deliveries. The role also requires communicating important information to the team, writing weekly client summaries as assigned, and maintaining accurate nursing documentation throughout the shift. This position also performs other related duties as needed. Division/Program Overview : Program: The Valley Star Crisis Walk-In Center (CWIC) is voluntary and offered on a 24/7 basis in an unlocked setting with a stay of up to 23 hours Population Served: All age groups – children, adolescents, adults, and older adults – who need immediate access to crisis mental health services. Facility & Setting: The Valley Star Crisis Walk-In Center (CWIC) is voluntary and offered on a 24/7 basis in an unlocked setting Clinical Services: Rapid mental health assessment and crisis intervention, the center provides important services to those in need of immediate crisis intervention and stabilization Length of Stay: stay of up to 23 hours Skills & Transition: Individuals admitted into the crisis stabilization unit will be provided with support and services to ensure their safe transition to the next level of care or back to the community. (Clinical nursing school rotations may be substituted for a portion of work experience.) One (1) year experience with seriously emotionally disturbed adolescents, in an inpatient health care setting, preferred. One (1) year leadership or supervisory experience preferred. How you will make a difference: Visually and audibly observes, documents in writing, and reports orally or in writing the physical or mental condition to the Director of Nursing in charge of the facility on the shift and the attending physician. Orders, prepares, administers, and documents the administration of medication for clients according to physician’s orders. Confirms delivery of all ordered medication from pharmacy on shift worked. Attends and participates in morning rounds, change of shift meetings, and treatment team meetings as scheduled on the assignment sheet. Monitors and maintains appropriate nursing documentation in the medical record for shift worked. Supports admission processes to ensure PHF / YPHF beds remain full, including reviewing admission packets and securing approval, scheduling admissions, and completing all required verifications & forms upon arrival in facility. Ensures timely and accurate attention to incidents and unusual occurrences. Provides direct nursing care and supervises same provided by all nursing staff the shift worked.
