This is a low-hour, flexible PRN opportunity (4–8 hours/week) ideal for a provider looking to supplement their schedule while making a meaningful impact in community-based mental health care. About the ACT Team Our ACTT program operates as a “hospital without walls,” delivering high-intensity behavioral health services directly in the community. We serve individuals with severe and persistent mental illness who are at high risk for psychiatric crisis and often face barriers to traditional care. You’ll work alongside a multidisciplinary team providing comprehensive, recovery-focused services that address mental health, substance use, housing, and overall stability. Essential Duties and Responsibilities: Properly communicate and guide LPNs in client care responsibilities and assignments. Complete initial Nursing Assessments for new admissions. Administer medications and be knowledgeable about withdrawal management protocols. Complete COWS and CIWA scales with appropriate clients per protocol. Provide medication education to clients. Coordinate and support medical providers during initial assessments and medication management appointments. Maintain medical equipment in proper working order. Complete required trainings and mandatory training hours. Perform other duties as assigned. Licensure, Education, & Experience: Current, unencumbered LPN license in Pennsylvania required. Current CPR/BLS and First Aid certification required. Background or experience in a behavioral health treatment setting preferred. Experience in Substance Use Treatment and Co-occurring Disorders preferred. This position serves as a key connection between nursing staff and leadership and helps coordinate staffing, clinical operations, and day-to-day patient care needs. The Charge Nurse also plays an important role in maintaining compliance with clinical, regulatory, and accreditation standards while promoting a structured, supportive environment for both clients and staff.
