Schedule/Shift : Mon-Fri 8AM-5PM Territory/Location: Salem and Surrounding location Responsibilities: Completes an initial assessment of the patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es). Initiates the plan of care and makes necessary revisions as patient status and needs change. Regularly re-evaluates patients’ needs. Uses health assessment data to determine nursing diagnosis. Develops a care plan which establishes goals, based on nursing diagnosis, incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. Counsels the patient and family in meeting plan of care and related needs. Provides health care instructions to the patient as appropriate per assessment and plan. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient. Qualifications Current and valid state RN license. Minimum of 1 year of recent acute care experience in an institutional setting. Basic Life Support (BLS), with hands-on CPR certification. Valid driver's license and auto insurance. Reviews and evaluates each case through a variety of means such as home visits, conferences, record review and the services provided by clinicians; discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services; and is available at all times during operating hours to assist clinicians as appropriate. Reviews patient's medical diagnosis, prognosis, medications, procedures, and clinical course. Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care. Uses health assessment data to determine nursing diagnosis. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. Provides health care instructions to the patient as appropriate per assessment and plan. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. Acts as Case Manager when assigned by Clinical Manager and assumes responsibility to coordinate patient care for assigned caseload. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilize opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group Qualifications: Graduate from an accredited school of nursing. Bachelor’s degree preferred. Current state license as a Registered Nurse. Minimum of 1 year of recent acute care experience in an institutional setting. Minimum of 1 year of recent experience in home care, preferably in hospice.
