This role is a 1099 contract opportunity, with care being provided at the patient’s residence. The current scheduling needs for this position will be established between the HHA/CNA, patient, and case manager. Shifts: Patients needs involve 24hr/day care - FT or PRN opportunities available, with FT shift times being 8a-8p and 8p to 8a. Offers are contingent upon passing a background check. Responsibilities Executes physician prescribed plans of care and compliant documentation of care provided in system of record. Administers prescribed treatments, and therapies. Performs patient assessments, coordination of care. Performs various activities associated with daily living, communications, and monitoring of vitals while working at the patient’s home.
