JOB TITLE: MDS COORDINATOR | RN JOB SUMMARY: Coordination and completion of Minimum Data Set (MDS), Admission Assessment, and Resident Care Plans. ESSENTIAL JOB FUNCTIONS : Document MDS in Electronic Charting System (ECS) in an accurate and timely manner. Complete daily MDS assessments (admission, quarterly, etc.) through Resident Interview Assessments on schedule. Update resident care plans (falls, wounds, infections, etc.) on a daily basis. Maintain the PPS and OBRA schedule. Monitor all members of the care plan team to ensure accuracy and completion of the MDS. Manage and schedule weekly care plan meetings. Meet with family members and interdisciplinary teams during the care plan meetings. Prepare and transmit MDS assessments to the state on a weekly basis. ESSENTIAL QUALIFICATIONS: Education and Experience: Must be a Registered Nurse in the State of Florida; Registered Nurse Assessment Coordinator (RNAC) Certification preferred. Assistant Director of Nursing (ADON)

Provides assistance to the Director of Nursing in planning, organizing, developing and directing the day-to-day operations of the Health Center in accordance with federal, state and local standards, guidelines and regulations. At the discretion of Administration, the Assistant Director of Nursing may be required to act as interim Director of Nursing in the event of their absence or vacation position. Develop and maintain nursing care objectives and standards of nursing care practices for this facility in cooperation with the Director of Nursing. Assist the DON by providing daily supervision of the unit managers, supervisors and ECS Coordinator. Conduct regular rounds to monitor resident activity and ensure quality resident care.
