Job Requirements POSITION SUMMARY: To be fully engaged in providing No Harm / Quality, Customer Experience, and Stewardship by: providing administrative support and assistance to the position of report and by providing outpatient assessment, diagnosis, treatment, consultation, and education as an advanced registered nurse practitioner, trained in a clinical specialty. PRIMARY ACCOUNTABILITIES: Quality/No Harm: Manages acute, episodic outpatient care and health maintenance needs of a dedicated patient population. Facilitates development, implementation, and review of standards or practice, policies and procedures, quality outcome indicators and other designated work projects to enhance care delivery. Customer Experience: Health Assessment may include basic physical. Interpretation of physical assessment findings and diagnostic results in determination of accurate differential diagnosis. Establishment of appropriate follow-up plan as necessary. Develops and implements formal and informal teaching plans for patients, families, staff, and community. Promotes patient/family-informed decision making in treatment plans and options. Stewardship: Participates in departmental CQI efforts and provides support for improvement of identified issues. OB Care Nurse Coordinator - Women Children

Job Requirements POSITION SUMMARY The Obstetric Care Nurse Program Coordinator provides comprehensive, patient-centered obstetrics (OB) and maternal care while leading the development, implementation, and ongoing coordination of maternal health programs across the care continuum. The Obstetric Care Nurse Program Coordinator functions as both a clinical navigator and program lead, supporting high-risk and underserved obstetric populations while advancing strategic initiatives that improve access, patient experience, and clinical outcomes. The Obstetric Care Nurse Program Coordinator serves as a liaison across clinical, operational, and community teams and supports organizational priorities including maternal health equity, reduction in Nulliparous, Term, Singleton, Vertex (NTSV) Cesarean (C-section) rates, and enhanced care coordination. PRIMARY ACCOUNTABILITIES Leads the development, implementation, and ongoing coordination of maternal health initiatives, including the Maternity onto Motherhood (MOM) Program, to improve access, patient preparedness, and care continuity across multiple campuses. Designs and standardizes prenatal education programming, including in-person, virtual, and group-based formats, to enhance patient experience and support organizational goals such as reduction in NTSV C-section rates. Develops and implements workflows for patient referral, engagement, and care coordination, ensuring timely access to services and continuity across outpatient, inpatient, and community settings. Collaborates with interdisciplinary teams, including clinical staff, Marketing, Community Benefit, Registration, and Health Information Management (HIM), to support program operations, patient access, and system integration. Acts as an Epic Care liaison for maternal health workflows, coordinating with Registration and HIM to create pregnancy episodes for patients receiving care outside of Epic-enabled practices and ensuring accurate input of critical prenatal data, including laboratory results, to support visibility of expected deliveries and continuity of care. Monitors and evaluates program performance, including patient engagement, barriers to care, and outcome trends, and contributes to performance improvement and maternal health equity initiatives. Identifies and addresses gaps in care delivery, particularly for high-risk and underserved populations, including non-English speaking patients to improve access and culturally competent care. Supports community clinics such as Health Department and Brevard Health Alliance (BHA) to coordinate timely and appropriate care delivery for high-risk and low risk maternal levels of care, including referrals, diagnostics, specialty services, and community resources. Serves as a central point of contact for patients and community partners, providing navigation, education, and support throughout the perinatal journey; supports performance improvement and maternal health initiatives, including collaboration with Perinatal Safety Nurse initiatives and other organizational priorities. Stewardship: ■ Accomplishes turnover in a timely manner, specific to facility by assisting circulator with cleaning and organization of supplies. ■ Prepares and organizes supplies and materials for the special needs of the patient, surgical procedure, and surgeon’s preference in a cost effective manner. Work Experience QUALIFICATIONS REQUIRED: ■ Graduate of program for Surgical Technologist (Military, Community College, or School for Surgical Technology) ■ Certification preferred ■ Three to five years of experience as a Surgical Technologist preferred ■ Must hold current AHA BLS Healthcare Provider Completion Card prior to start date and maintain. PHYSICAL DEMANDS: ■ Exposure to bio hazardous waste, substances, and unknown diseases. ■ Vision and hearing to normal range. ■ Sitting and standing for extensive periods of time. ■ Frequent lifting, pulling, reaching, and carrying of light to heavy patients and/or objects. ■ Must be able to lift up to 40 pounds unassisted up to 33% of the assigned shift. MENTAL DEMANDS: ■ Working under stressful situations. ■ Continuous concentration on an activity. ■ Ability to understand and follow written and verbal instructions. ■ Interacts effectively with members of the medical staff and patient care team.
