Any staff member requiring limited access to secured medication rooms must have a fully completed and signed Medication Security Agreement for Non-Licensed Personnel form on file and available upon request. Responsibilities: Follows nursing assistant plan of care established by the Registered Nurse within a specified timeframe; including, but not limited to, mouth care, bathing, AM and PM care, assistance with ambulation, transfers to chair and/or bed, taking and documenting vital signs, and recording accurate intake and output. Documents all patient care delivered according to established policies and procedures. Performs duties with the knowledge and skills necessary to provide care appropriate to the age of patients served on the assigned units. Maintains a safe, comfortable, and therapeutic environment for patients and families in accordance with hospital standards. Communicate relevant patient information to a professional nurse within an appropriate time based on the level of urgency. Report problems immediately to a professional nurse. Answer intercom or call lights in a timely, courteous manner, with appropriate communication to the patient and/or professional nurse. Performs patient care based on established guidelines for diagnostic and therapeutic exams; i.e., NPO for labs or x-rays, OR, diet changes related to tests, enema-related tests. Perform safety and suicide precaution sitter duties as assigned. Assists other members of the nursing team and/or ancillary departments as requested. All other duties as assigned, within the scope and range of job responsibilities. Required Education, Credential(s) and Experience: Education: High School Diploma or GED; Must meet one or more of the following criteria: Completion of a Nursing Assistant training course, Medical Assistant training course or Patient Care Technician training course. The care may include, but is not limited to, the following: obtaining and recording in the medical record: blood pressure, pulse, temperature and pulse oximetry, drawing blood, EKG, administration of oral medications, transporting patients, obtain urine and stool specimens, perform point of care testing, assist with procedures consistent with the scope of the employee’s position (i.e.. dressing changes, dressing, grooming, toileting, ambulating, transferring, observing, and reporting the general well-being of the person(s) to whom they are providing care). Responsibilities: The LPN must read and interpret documents such as doctors’ orders, medication names, and lab tests. Perform focal assessments for established inpatients. Provide general basic nursing care, such as positioning patients, lifting and turning, assisting in use of bedpan, commode, urinal or bathroom, and weighing. Provide exceptional customer service that aligns with the Planetree philosophy. Perform routine, uncomplicated treatment procedures including enemas, application of hot and cold compresses, dressing changes, wound cleansing, obtaining vital signs including orthostatic vital signs and rectal temperatures and splinting of extremities. Responsibilities include conducting various cardiac tests to monitor patients' heart and blood vessel conditions using diverse cardiology equipment, preparing procedure rooms, prepping patients for procedures, obtaining, and recording patient information, assisting physicians in procedure by circulating procedures, X-ray fluoroscopy proficiency, performing blood work analyses, and operating intra-cardiac ultrasound. Directs and practices safety guidelines for themselves, staff, and patients. Independently functions during diagnostic and interventions procedures.
