Supervise, mentor, and support nursing staff to ensure high-quality resident care. Monitor resident conditions and ensure timely assessments, documentation, and follow-up. Collaborate with physicians and the interdisciplinary team to develop and implement individualized care plans. Ensure compliance with federal, state, and facility policies and nursing standards. Assist with admissions, transfers, and discharges. Review physician orders, medication administration records, and clinical documentation for accuracy. Address resident, family, and staff concerns in a timely and professional manner. Participate in quality assurance initiatives, infection prevention activities, and regulatory surveys. Participate in the facility's on-call rotation to provide clinical leadership and support outside of regular business hours. Qualifications Current, active Pennsylvania LPN or RN license. Previous experience in a skilled nursing, rehabilitation, or long-term care setting. Previous experience as a Charge Nurse, Unit Manager, or other nursing leadership role. Strong leadership, communication, and organizational skills. Ability to effectively manage multiple priorities in a fast-paced environment. Willingness to participate in an on-call rotation .
