Qualifications for Licensed Practical Nurses (LPN): Valid Licensed Practical Nurse (LPN) license in PA Previous experience in a skilled nursing facility, rehabilitation center, long-term care, or healthcare setting for at least 6 months. CPR Certification Compassionate, patient-focused, and committed to providing high-quality care. Ability to work independently and collaboratively within a team. Excellent communication and interpersonal skills. Ability to maintain accurate records and handle sensitive information confidentially. Pre-Hire Requirements Current physical examination completed within the past 2 years Completed PPD/TB test Current CPR certification required Completed background check Current nursing license Responsibilities: Administer medications and treatments as prescribed by physicians. Monitor and document residents' health conditions, including vital signs, symptoms, and responses to treatments. Assist in the development and implementation of individualized care plans. Provide nursing care for residents, including wound care, catheter care, and assistance with daily living activities. Collaborate with physicians, RNs, and other healthcare team members to ensure coordinated and comprehensive care. Educate residents and their families about health management and procedures. Ensure that all care is delivered in accordance with regulatory and facility guidelines. Respond to emergencies and provide critical care when necessary. The RN Supervisor will provide hands-on nursing leadership, support quality resident care, and help ensure the nursing department operates safely, efficiently, and in compliance with applicable regulations.
