Minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. Nurse Practitioner- Palliative/Hospice Care Full Job Description Primary Duties and Responsibilities The Home Health- Nurse Practitioner is responsible for assisting in the expedient multidisciplinary care of patients for Hoag Hospital and its affiliates, coordinating seamless patient care from admission through discharge in the Home Health setting. Providing supplemental medical care to patients with a focus on quality and patient experience, the Nurse Practitioner can perform history and physical exams, diagnoses and treats medical problems, orders and interprets diagnostic and laboratory tests, performs minor procedures and facilities the discharge process.
