Serenity Hospice PA is seeking an experienced Hospice RN Case Manager for a dedicated facility-based position in West Chester, Pennsylvania. This is a unique opportunity to case manage hospice patients while being assigned to just one facility . Instead of driving throughout a large territory or traveling between multiple facilities each day, you will report to one West Chester location , allowing you to spend more of your day focused on patients, families, and clinical care. You will still have the responsibilities of a true Hospice RN Case Manager, you'll manage your patients, coordinate their care, assess changing needs, communicate with families and facility staff, and collaborate with the hospice interdisciplinary team. ABOUT THE ROLE As the dedicated Hospice RN Case Manager for this West Chester facility, you will serve as Serenity Hospice's primary clinical presence at the location and oversee the hospice care of patients residing there. This is not an inpatient hospice unit . Patients reside within the facility and receive hospice services there. You will build strong relationships with patients, families, facility nurses, administrators, caregivers, physicians, and the hospice interdisciplinary team. The ideal candidate will become a trusted clinical leader and resource within the facility .
