As a compassionate and dedicated Licensed Practical Nurse (LPN), you will play a vital role in delivering exceptional care to our residents. Job Summary: The Licensed Practical Nurse (LPN) provides direct nursing care to residents in accordance with physician orders, facility policies, state and federal regulations, and accepted standards of nursing practice. The LPN works under the supervision of a Registered Nurse and collaborates with the interdisciplinary team to ensure quality resident care and services. Responsibilities: Administer medications and treatments as prescribed by physicians. Monitor residents' health status and report significant changes to the physician and supervising nurse. Provide wound care and other clinical interventions Perform nursing assessments within scope of practice. Ensure accurate and timely documentation is informative and in a descriptive manner. Admit, discharge, and transfer residents. Supervise CNAs to ensure adherence to care standards in accordance with current rules, regulations, and guidelines Maintain infection prevention and control practices Support implementation and updates of individualized care plans for each resident. RN Supervisor

RN Supervisor Job Summary: Washington Rehabilitation and Nursing Center, a leading provider of skilled nursing and rehabilitation services, is seeking an experienced and dedicated RN Supervisor to lead our night shift (7:00 pm-7:00 am) in Chipley, Florida. As a Registered Nurse (RN) Supervisor , you will oversee the nursing staff, ensure quality resident care, and maintain compliance with all state and federal regulations during the assigned shift. Key Responsibilities: Supervise nursing staff on evening shifts, providing guidance, mentorship, and support to ensure optimal patient care delivery. Supervise all nursing personnel during night shift, including assigning and distributing work assignments, performance management, and training new staff. Oversee medication, IV infusions, catheter care, wounds, and other clinical procedures following aseptic technique and infection control protocols. Manage patient intake and discharge planning processes, including coordination with case management and utilization review teams to optimize patient outcomes. Facilitate communication among multidisciplinary teams, including physicians, therapists, social workers, and ancillary staff to coordinate comprehensive patient care plans. Monitor clinical competencies to ensure accuracy and completeness for all staff. Oversee scheduling to ensure sufficient number of staff is available for each shift to ensure that quality care is maintained. Lead incident investigations related to patient safety events or infection control breaches; implement corrective actions promptly. Qualifications: Education/Experience: Must possess a current, unencumbered, active state license to practice as a RN, CPR/BLS certified, minimum 4+ nursing experience in a nursing home or other healthcare settings, and a minimum 1-year experience as a nurse supervisor. Strong knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care. Supervisory Requirements: Supervises the nursing personnel and the day-to-day nursing activities of the facility during the assigned shifts.
