We are seeking a caring and dedicated Weekend RN Supervisors for all shifts to join our rapidly growing team at our skilled nursing facility in West Babylon! Job responsibilities include but are not limited to: The ideal candidate will oversee the total care and safety of all residents and employees for assigned unit during shift Supervises and assists all nursing personnel in their work performance when necessary Performs duties of Unit Nurse when necessary Document patient histories, administers medication and care as needed, coordinate necessary appointments Monitor residents to ensure that the plans are being followed and that the resident’s and family needs are being met Plan, coordinate, and direct all nursing staff on all shifts on the assigned units Minimum Qualifications include: Currently licensed as Registered Nurse in New York State Minimum 2 years supervisory experience in a long-term health care setting Strong leadership and management skills Has knowledge of Federal and State health regulations Experience with care plans and strong documentation skills Flexibility in work hours BLS Certification RN License The shifts are Morning (7am to 3pm) Evening (3pm to 11pm) Overnight (11pm to 7am)

SCHEDULE: 3pm-7pm 5 days per week with rotating weekends. Berkshire Center is a skilled nursing facility located in West Babylon, NY. Our mission is to provide the highest level of care for each of our residents. We are looking for an experienced LPN that can provide a high level of care. This is a union position. JOB DUTIES: The purpose of your job as a Licensed Practical Nurse is to perform a variety of care practices under the direction of the Registered Nurse and/or Supervisor of the unit/shift. This includes, but not limited to: Take and record measurements of blood pressure, temperature, heart rate etc. Observe residents under treatment to identify progress, side-effects of medications etc. Monitor residents’ condition including fluid intake and output and compose patient charts. Administer injections, prescribed medications, enemas etc., clean and dress wounds. Collect samples for lab testing as assigned. Provide emotional and psychological support when needed. Communicate with residents’ family or friends to provide advice, comfort, and release instructions. QUALIFICATIONS: Please do not apply if you cannot meet the shift requirements (5 days per week and must be able to arrive at 3pm) Nursing home or LTC experience required Currently licensed by the State of New York.. This is an excellent opportunity for an RN looking to move into a nonclinical role. Job responsibilities include but are not limited to: Completing accurate assessments, MDS & care plans as assigned. Monitors MDS and care planning documentation for all residents; ensures documentation is present in the medical record to support MDS coding. Initiating care plans and supporting activities as assigned. Maintaining & updating all care plans and assessments as required. Monitoring & auditing clinical records, ensuring accuracy & timeliness.
