Overview This individual is a clinically competent Cardio-Vascular Advance Practice Clinician (APC) will directly report to the Director of APCs with a matrix reporting structure to the Executive Director of the Cardiac and Vascular Service Line. S/he must be able to effectively deliver care through demonstrated expertise to the Cardio-Vascular patient. The principle duties and responsibilities include but are not limited to: performing pre-procedure assessments, Histories and Physicals, education and counseling of patients on cardiac and vascular diseases and treatments, responsible for tentative diagnosis, order and provide preliminary interpretation of lab studies, post procedure management of patient, discharge patient. Responsibilities Responsibilities Provides timely and thorough pre procedural work up on all Cardio-Vascular patients to include orders, History and Physical, review of consent and lab tests. Provides post procedural orders, discharge planning and education on all Cardio-Vascular patients. Provides direct patient care to Cardio-Vascular patients and evaluates patient outcomes. Participates with evaluation and eminence of closure devices, sheath pulls and arteriotomy sites. Performs orders and interprets results of diagnostic and lab tests on all pre and post cardio-vascular patients. Provides training and education to the CCL Staff, ED and inpatient units, staff and community as needed. Actively participates in mock codes and cardiac arrests on cardiac units. Actively participates in review of PI data and all PI activities associated with the cardio-vascular patient Assessment & Management Assesses for risks associated with the care of the acute and complex chronically ill patient including: medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions and diagnostic procedures. Orders and collects data using appropriate evidence-based assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated. Assesses for risks associated with the care of the acute and complex chronically ill patient including: medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions and diagnostic procedures. Diagnosis Collaborates with interdisciplinary health care team in making diagnoses of acute and complex chronic conditions. Manages diagnostic tests through ordering, interpretation, performance, and supervision. Formulates differential diagnoses by priority. Diagnoses complications and orders appropriate interventions Formulates Plan of Care Identifies expected outcomes from diagnosis, and formulates and documents a plan of care to address complex acute and chronic health care needs. Utilizes evidenced based practice guidelines and protocols in an individualized, dynamic plan of care that can be applied across the continuum. Implements and modifies plan of care. Prescribes diagnostic strategies and therapeutic interventions. May perform advanced procedures consistent with privileges and competency validation. Communication and Collaboration Maintains ongoing communication and collaboration with interdisciplinary health care team. Facilitates and communicates with patient, family and staff to promote continuity of care across the inpatient and outpatient continuum of care. Documentation Documents/dictates key components of patient's progress via daily progress note, transfer, and discharge summary, and or clinic note where applicable. (h&p, daily progress notes, plan of care, problem lists, procedure notes, acute event note, discharge summaries, in medical record per specific patient unit or service, death summary) Documentation is timely, meets acute care compliance standards and captures patient acuity. Professional Practice Demonstrates Professional Practice behaviors including: preceptor/mentoring, education and instruction of graduate students. Seeks opportunities for active engagement in research and the analysis of evidenced based practice. Actively participates in Advance Practice Grand Rounds and specialty specific meetings. Maintains CEUs, and membership in a professional organization. Qualifications Minimum 3 years experience in clinical nursing. Minimum 2 years experience functioning as an APC in the critical care cardio-vascular setting, preferred settings include but are not limited to the Cardiac Catheterization Lab, Non-invasive stress testing, and Vascular procedural area. Must have strong clinical background with strong communication and motivational skills. Must have knowledge of pharmacology principles, dysrhythmia, ability to assess clinical situations and initiate appropriate treatment and interpret laboratory data, respond to emergency situations, coordinate work schedules to achieve maximum productivity and efficiency. ADVANCED PRACTICE CLINICIAN (APC) Medical Oncology - Nurse Practitioner

Overview This individual is a clinically competent, certified advanced practitioner, who serves in the comprehensive care of the medical oncology and hematology patient throughout the continuum of care. Under the scope of an advanced practice clinician and approve privileges within Beebe Healthcare, the individual is expected to perform comprehensive patient assessments, follow-up care, diagnosis, side effect and infusion reaction management, survivorship care services, and patient education/counseling in collaboration with the medical oncology physicians. Follow(s) established advanced practice standards, procedures, and guidelines/protocols to give direction to patients, families, nursing and other staff when appropriate. Responsibilities Performs comprehensive assessments for new and established oncology and hematology patients. Management of complex oncology and hematology patients to include side effect management, diagnosis, and treatment. Demonstrates critical thinking, prioritization of duties, and time management skills based on patient assessment and interpretation of test results. Serves as the primary clinical support for the infusion center when assigned. Duties include management of infusion reactions, patient care needs presented by nursing and pharmacy, coverage of triage issues and assessments, and critical lab and radiology results. Formulate and implement survivorship care plans for all curable cancer patients. Identify and initiate referrals to palliative care. Collaborate with the palliative care team for management of patient's ongoing care. Communicates positively and effectively with patients, families, physicians and the multidisciplinary oncology team. Coordinates patient care with PCP and specialty providers when appropriate. Provides detailed education and counseling to patients and families regarding diagnosis, goals of therapy, expectations, and treatment related side effects and management. Develop a comprehensive plan of care in collaboration with the attending oncologist to include selection of appropriate pharmacologic agents, referrals, and end of life care. Inpatient oncology and hematology acute care coverage to include rapid assessment and treatment of acutely ill oncology/hematology patients. Collaborate with medical oncologist and inpatient disciplines to determine the most appropriate level of care. Assist in the admission and discharge of patients receiving inpatient chemotherapy in collaboration with the attending oncology physician. Administration of intrathecal chemotherapy in collaboration with interventional radiology. Provide after-hours call coverage as part of a rotating schedule to include phone triage, clinical decision-making, and urgent inpatient/outpatient concerns in conjunction with on-call oncologist. Other duties as assigned Qualifications Education/Experience Graduate from an accredited Physician Assistant or Nurse Practitioner program. Previous experience in oncology as provider or nursing preferred. Licensure/Certification Licensure as an advanced practice clinician (PA or NP) in the state of Delaware or ability to obtain with offer letter.
